摘　　要：目的应用组织多普勒成像(TDI)检测右房室瓣环舒张期运动速度,评价原发性高血压不同左室构型时的右室舒张功能。方法高血压患者80例按左室重量指数和相对室壁厚度的不同,分为正常左室构型组(A组,21例)、向心性重构组(B组,22例)、向心性肥厚组(C组,19例)、离心性肥厚组(D组,18例),TDI测量右房室瓣环舒张早期峰速度(e)、舒张晚期峰速度(a),多普勒测量右房室瓣血流E波减速时间(EDT)、右室等容舒张时间(IVRT),并与30例健康者(对照组)对比。结果与对照组比较,B、C、D组e波及e/a比值下降(P均<0.01),且e波呈递减趋势;B、C组a波增高(P均<0.01);B、C、D组EDT、IVRT增高(P均<0.01)且呈递增趋势。结论高血压所致左室构型改变影响右室舒张功能。Objective To evaluate right ventricular diastolic function in essential hypertension patients with different left ventricular geometry using tricuspid annular motion detected by tissue Doppler imaging（TDl）. Methods According to left ventricular mass index and relative wall thickness,eighty hypertension patients were divided into normal geometry group（A group,21 cases）,concentric remodeling group（B group,22 cases）,concentric hypertrophy group（C group, 19 cases） and eccentric hypertrophy group（I） group, 18 cases）. The tricuspid annular motion diastolic early peak velocity（e peak）,diastolic late peak velocity（a peak）,E-peak deceleration time（EDT） and isovolumic relaxation time（IVRT） were measured. C, omparisons among ones with control group were performed. Results Compared with control group,e peak and e/a ratio decreased in B,C,D groups（P all 〈 0.01 ）, In B,C groups,a peak were higher than that of control group（P all 〈 0.01 ）. EDT and IVRT were increased in B, C, D groups（P all 〈 0. 01 ）. EDT, IVRT, e peak showed increased or decreased tendency with different patterns of left ventricular geometric models. Conclusion With left ventricular geometry changed,right ventricular diastolic function is decreased to different degrees in hypertension patients.
