摘  要:目的探讨外周血干细胞采集过程中出现不良反应的预防和处理.方法对24例患者或供者,使用CS-3000 Plus血细胞分离机采集外周血干细胞,对采集过程中出现的不良反应进行观察和分析.结果每例采集2~3次,共58次,出现不良反应14次(24%),其中以枸橼酸盐毒性反应最多(13次),发热反应1次,经采取相应处理措施后,不良反应消失.24例均能采集到足量的单个核细胞,顺利完成移植.结论外周血干细胞采集过程中可能出现不良反应,以枸橼酸盐毒性反应最多,处理不当,采集难以顺利进行.为确保采集成功,必须做好采集前准备、控制处理血液总量(终点量值)、全血流速及抗凝剂比例,并注意预防和及时处理.Objective To investigate the methods of prevention and cure for the adverse reactions in the collecting procedure of peripheral blood stem cells (PBSC). Methods Twenty-four patients or donors were observed on adverse reaction in collection of PBSC by blood cell separator (CS-3000 Plus). Results The adverse reaction occurred in 14 of 58 collections (24. 14% ), 13 of citrate poisoning and one of febrile reaction, and disappeared after treatment. All collections of PBSC were completed and all patients were successfully transplanted with peripheral blood stem cells. Conclusion Our study suggests that the most of adverse reactions in the collection of PBSC were citrate poisoning. The pre-collection preparation, the appropriate control in end-point volume, whole blood flow rate and the mixing proportion of whole blood with anticoagulant ( WB : ACD) were required.
