曲马多鞘内预注对大鼠足底皮下注射福尔马林的超前镇痛效应

摘　　要：目的:观察鞘内预注曲马多对大鼠足底注射福尔马林后伤害性效应的影响,探讨曲马多在脊髓水平的超前镇痛效应。方法:64只W istar大鼠蛛网膜下腔埋入导管并长期留置,实验时经导管将药物注入蛛网膜下腔;于大鼠左后爪掌心皮下注射2.5%福尔马林50μl建立伤害性动物模型。实验分为A、B两大组,A组于福尔马林注射前5m in给药;B组于福尔马林注射后15m in给药。A、B两组分别再分为四组(每组8只大鼠):即生理盐水组,曲马多1、5、10μg组;以大鼠福尔马林注射后第二期反应的疼痛行为观察指标,并进行Dubuisson&Dennis评分,观察注药后的反应。结果:无论在福尔马林注射前或后给药,曲马多均能产生量效、时效相关的抗伤害作用,ED50分别为:3.68μg和5.08μg;95%置信区间分别为(1.34,6.29)和(2.27,7.92)μg。福尔马林注射前鞘内注入5、10μg曲马多的抗伤害效应评分在福尔马林注射后的第35、40、45m in时低于福尔马林注射后鞘内注入相同剂量曲马多的评分(P<0.05)。结论:在脊髓水平,曲马多能产生量效、时效相关的抗伤害作用,且具有超前镇痛效应。Objective： To observe the preemptive effect of intrathecal tramadol on the formalin induced response in rats. Methods： Wistar rats with intrathecal catheter were randomly divided into two main groups： A and B, each containing four subgroups, which were saline, 1 μg tramadol, 5 μg tramadol, and 10 μg tramadol group, respectively. Animal models were made by subcutaneous injecting 2.5% formalin （50 μl） into the plantar surface of the left hind paw of rats. Group A received saline or tramadol 5min before formalin injection and group B 15min after fomalin injection. Nociceptive behaviors were tested between 30 and 60 rain after formalin injection according to Dubuisson ＆ Dennis Scores. Results： For both Group A and B, intrathecal administration of tramadol（1～10 μg） produced significant dose-dependent and time-dependent antinociception to the second phase reaction. ED50 were 3.68 μg and 5.08 μg, respectively; 95% confidence intervals were （1.34, 6.29） μg and （2.27, 7.92） μg, respectively. Antin-ociception score of group A was less than that of group B （5, 10 μg, i.t.） at 35, 40 and 45min（P 〈 0.05）. Conclusion： Intrathecal administration of tramadol can produce time-dependent and dose-dependent antinociceptive effect. Tramadol has preemptive analgesic effect at the spinal level.
