足月新生儿脐血瘦素水平测定及其与代谢类激素间的关系研究

摘　　要：目的探讨瘦素对胎儿生长发育的影响及其与代谢类激素的关系.方法采用放射免疫分析法对河北医科大学第一医院2003～2004年出生的83例足月新生儿进行瘦素、胰岛素(RI)、生长激素(GH)、促甲状腺激素(TSH)、甲状腺激素(T3、T4)测定,测量新生儿出生体重、身长、记录性别及分娩方式并计算体重指数(BMI),评估新生儿营养状态.结果 (1)足月新生儿脐血瘦素质量浓度为(10.53±5.04)μg/L,性别间差异无显著性(P>0.05);不同分娩方式间差异无显著性(P>0.05).(2)新生儿脐血瘦素质量浓度与体重指数呈正相关(r=0.5196,P<0.01).瘦素质量浓度在大于胎龄(LGA)儿、适于胎龄(AGA)儿、小于胎龄(SGA)儿3组间差异有显著性(P<0.01),AGA儿组明显低于LGA儿组而高于SGA儿组.(3)新生儿脐血瘦素质量浓度与GH呈正相关(r=0.5946,P<0.01),与RI呈正相关(r=0.4364,P<0.05),与TSH呈负相关(r=-0.4268,P<0.05),与总T3、总T4呈正相关(r分别为0.4472、0.3571,P均<0.05).结论瘦素对胎儿宫内生长发育起重要的调节作用,与代谢类激素关系密切.Objective To measure the leptin concentration in umbilical serum of newborns ,to explore its effects on fetal development and to study its relationship with metabolic hormones RI.GH.TSH.TT3 ,TF4, Methods Levels of umbilical venous leptin and metabolic hormones were measured in 83 newborns from the First Affiliated Hospital of Hebei Medical University in 2003 - 2004 by immmunoradioassay method. Neonatal body weight, body length.gender and mode of delivery were recorded, and body mass index was calculated to evaluate [ BMI = （ kg/m^2 ） ] neonatal nutrition state. Results The full-term neonatal umbilical serum leptin level was （ 10, 53 ＋ 5, 04 ） μg/L, there was no significant difference with gender（ P 〉0. 05） or the mode of delivery（ P 〉0, 05）, Neonates umbilical serum leptin concentration was positively correlated with BMI（ r =0. 5196. P 〈0, 01 ）, There were significant differences among three groups（ P 〈0, 01 ） .leptin level in AGA was significantly higher than SGA group but lower than LGA group, Neonates umbilical serum leptin concentration was positively correlated with RI （ r = 0. 4364, P 〈 0. 05 ） , GH （ r = 0. 5946, P 〈 0. 01 ） , TT3 （ r = 0. 4472, P 〈 0.05） ,TT4（ r =0.3571, P 〈0.05） ,but negatively correlated with TSH（ r = -0.4268P 〈0.05）.Conclusion Leptin regulates fetal intrauterine development. It has close relationship with other metabolic hormones.
