椎管侧前方减压治疗外伤性截瘫(附17例报告)

摘　　要：本文报道17例早、晚期截瘫患者,施行椎管侧、前方减压术,获满意效果。Seventeen cases of spinal cord injury caused by fracture of cervical, thoracic or thoraco-lumbar vertebrae （from 1983 through 1985） were treated by antero-lateral decompression of the vertebral canal and fixation with bone fragments from iliac crest. Among these patients ten were complete and seven were incomplete paraplegic. All were caused by the backward compression on the spinal cord by the displaced fragment of the posterior part of the vertebral body and intervertebral disk All cases were followed for an average of 12 months（range 9-24 months）. The motor and sensory functions were significantly improved. In four cases, the strength of the quadrecips and popliteal and string muscles returned to such a degree that the patients were able to walk In one case, the normal control of the bladder and the bowel was regained. No aggravation of the symptoms were seen after the operation The result of early antero-lateral decompression of the vertebral canal is satisfactory, thorefore, this procedure is highly recommanded in the treatment of traumatic injury of the spinal cord.
