摘 要:目的:调查并分析延边农村社区汉族和朝鲜族人群血压及其影响因素的分布特征。方法:①于2004-08/09从延边地区汪清县大兴沟镇内采取单纯随机原则抽取50~60岁汉族和朝鲜族人群458人。纳入对象的来源地区民族比例相近,且对调查内容均知情同意。②于2004-08/09在被选社区及其卫生院,采用家访询问的方法和健康体检的形式收集本次调查相关信息。③调查前对所有调查人员严格培训,规范调查内容、操作流程和方法。并按统一的调查表(内容包括研究对象的一般情况、心脑血管疾病的既往史及家族史等)进行家访询问调查。④测量身高、体质量、腰围、臀围、血压等。⑤取清晨空腹(禁食12h)静脉血,测量血清总胆固醇、三酰甘油、高密度脂蛋白胆固醇、空腹血糖等指标。计算动脉硬化指数(总胆固醇-高密度脂蛋白胆固醇)/高密度脂蛋白胆固醇。上述生化指标均用全自动生化分析仪测定。⑥计量资料差异比较采用t检验;血压与其影响因素的关系分析采用Logistic回归分析;以高血压为因变量,以民族、性别、总胆固醇、三酰甘油、高密度脂蛋白胆固醇、高密度脂蛋白胆固醇/总胆固醇、动脉硬化指数、空腹血糖、体质量、体质量指数、腰围、腰臀比为自变量进行Logistic回归分析,BACKWARD法选择变量,以α=0.05为入选变量的显著性水准,α=0.1为删除变量的显著性水准。结果:调查问卷、体格及生化指标检测结果完整者455人,男183人(汉族114人、朝鲜族69人),女272人(汉族183人、朝鲜族89人)。①朝鲜族人群高密度脂蛋白胆固醇水平和体质量明显低于汉族(P<0.01),而收缩压、舒张压、空腹血糖明显高于汉族(P<0.01),男性朝鲜族人群动脉硬化指数明显高于汉族(P<0.05)。②多因素分析结果表明,最终被选入方程的因素分别是:单纯高收缩压为民族、性别、三酰甘油、高密度脂蛋白胆AIM: To survey and analyze the distributing characteristics of blood pressure and its influencing factors among Han nationality and Korean-Chinese in countryside community of Yanbian. METHODS: ① Using simple randomized principia, 458 people of Han nationality and Korea-Chinese aged 50-60 years from August to September 2004 in the Daxinggou town, Wangqing county of Yanbian were selected. The original area and national proportion of people brought into were close, and they knew and agreed the surveying matters. ② The surveying related information was collected by visiting home enquiry and health examination in selected community and public health center from August to September 2004. ③ The surveyors were strictly trained before investigation and the inventory content, operative procedure and methods were standardized, and it was carried through to visit home and inquiry with a unitive questionnaire including general status, previous history and family history of cardiovascular diseases, and so on, ④ The height, body mass, waistline, hipline and blood pressure etc. were measured. ⑤ The fasting vein blood in early morning (12 hours of fasting) was taken out, and the serum total cholesterol, triglyceride, high-density lipoprotein cholesterol (HDL-C), and fasting blood-glucose etc. were measured. Arteriosclerosis index (total cholesterol-HDL-C)/ HDL-C was calculated. The upper biochemical indexes were detected with the whole automatic biochemistry analyzer. ⑥ The measurement data diversity was compared with t-test; relationship between the blood pressure and its influencing factors was analyzed with the Logistic regression analysis that was performed with hypertension as dependent variable, and nationality, sex, total cholesterol, triacylglycerol, HDL-C, HDL-C/total cholesterol, arteriosclerosis index, fasting bloodglucose, body mass, body mass index, waistline, waist-hip ratio as independent variable, and the variable was selected by the BACKWARD method in which α= 0.05 as significant leve
