摘 要:目的:观察选择性环氧化酶-2抑制剂罗非昔布及环氧化酶非特异性抑制剂阿司匹林对心肌急性缺血兔血浆内血栓素B2/6-酮-前列腺素F1α比值与心肌细胞凋亡率的影响。方法:①实验于2001-08/2002-05在解放军总医院急诊科完成。选用雄性成年清洁级新西兰兔28只。随机分为4组:正常对照组、心肌缺血对照组、心肌缺血+罗非昔布组、心肌缺血+阿司匹林组,每组各7只。②正常对照组未造成心肌缺血模型,其余3组均造成心肌缺血模型。心肌缺血+罗非昔布组和心肌缺血+阿司匹林组于心肌缺血模型制作前7d开始灌胃罗非昔布1mg/(kg·d)、阿司匹林5mg/(kg·d),共干预7d;正常对照组和心肌缺血对照组每日给予等量生理盐水灌胃,干预7d。③于兔冠状动脉左前降支第一穿支与第二穿支之间7-0号丝线缝扎前降支动脉造成急性心肌缺血模型。左前降支结扎6h后,留取心肌缺血区与正常心室标本。④心肌环氧化酶-2表达检测采用免疫组织化学法;心肌细胞凋亡阳性率检测采用原位末端标记法检测;血浆6-酮-前列腺素F1α、血栓素B2水平检测采用放射免疫法检测。⑤计量资料差异性比较采用t检验。结果:兔28只均进入结果分析。①血浆血栓素B2水平:各心肌缺血组明显高于正常对照组(t=2.69~11.42,P<0.05);心肌缺血+罗非昔布组明显低于心肌缺血对照组(t=7.74,P<0.05),明显高于心肌缺血+阿司匹林组(t=9.45,P<0.05)。②血浆6-酮-前列腺素F1α水平:各心肌缺血组明显高于正常对照组(t=5.69~12.46,P<0.05);心肌缺血+罗非昔布组明显低于心肌缺血对照组和心肌缺血+阿司匹林组(t=10.39,4.24,P<0.05)。③血栓素B2/6-酮-前列腺素F1α:心肌缺血对照组、心肌缺血+罗非昔布组明显高于正常对照组(t=3.54,7.07,P<0.05),心肌缺血+阿司匹林组明显低于正常对照组(t=7.25,P<0.05);心肌缺血+罗非昔布组明显高于心肌缺血对照组和心肌缺血+�AIM: To observe the effects of selective cyclooxygenase-2 inhibitor (rofecoxib) and non-selective cyclooxygenase inhibitor (aspirin)on the ratio of thromboxane B2/6-keto-prostaglandin F1α, (TXB2/6-keto-PGF1α ratio) in plasma and myocardial apoptosis in rabbits with acute myocardial ischemia. METHODS: ① The experiment was carried out in the emergency department, General Hospital of Chinese PLA from August 2001 to May 2002. Male New Zealand rabbits (n=28) of clean degree were randomly divided into four groups: normal control group (n =7); myocardial ischemia control group (n=7); myocardial ischemia + rofecoxib group (n =7) and myocardial ischemia + aspirin group (n=7). ② Besides the rabbits in the normal control group, those in the other 3 groups were made into models of myocardial ischemia. Gastric perfusion of rofecoxib (1 mg/kg per day) and aspirin (5 mg/kg per day) were given in the myocardial ischemia + rofecoxib group and myocardial ischemia + aspirin group respectively for 7 days before model establishment, and those in the normal control group and myocardial isehemia control group were treated with gastric perfusion of saline of the same volume every day for 7 days. ③ Models of acute myocardial ischemia were made by ligating anterior descending artery with 7-0 size thread between the first and second perforating branches, and samples of myocardial region and normal ventricle were taken at 6 hours after ligation. The expression of cyclooxygenase-2 was detected with immunohistochemistry, the positive rate of myocardial apoptosis was detected with in situ nick end-labeling (TUNEL), the levels of 6-keto-PGF1α and TXB2 in plasma were determined with radioimmunoassay. ⑤The difference of the measurement data was compared with the t test. RESULTS: All the 28 rabbits were involved in the analysis of results. ① The plasma level of TXB2: It was obviously higher in the myocardial ischemia groups than in the normal control group (t=2.69 to 11.4
