益肾清利活血综合疗法治疗局灶节段性肾小球硬化及IgA肾病患者64例临床观察

摘　　要：目的:研究益肾清利活血法治疗局灶节段性肾小球硬化(FSGS)和 IgA 肾病(IgAN)的临床疗效。方法:以经肾穿制活检确诊的 FSGS 和 IgAN 患者为研究对象,采用定期门诊复诊的方法,3个月为1个疗程,观察益肾清利活血综合疗法对24小时尿蛋白(UTP)及血肌酐(SCr)水平的影响。结果:FSGS 组及 IgAN 组治疗6个月、12个月时总有效率分别为80.0%与82.4%,83.3%与85.3%,两组间疗效差异无显著性。经重复测量的方差分析显示,FSGS 组治疗15个月后 UTP 下降明显(P<0.05),IgAN 组治疗3个月后 UTP 即明显下降(P<0.05),但两组间差异无显著性。两组治疗第6个月、9个月时 SCr 与治疗前比较差异均有显著性(P<0.05),但治疗12个月、15个月时 SCr 与治疗前比较差异无显著性(P>0.05)。治疗后 UTP 及SCr 各时间点之间比较差异无显著性。结论:益肾清利活血法长期综合治疗可以改善症状,降低 UTP,稳定肾功能。Objective： To observe clinical therapeutic effects of the method of tonifying the kidney, removing dampness and activating blood circulation on focal and segmental glomerular sclerosis （FSGS） and IgA nephropathy （IgAN）. Methods： 64 out-patients were diagnosed as FSGS（n=30） and IgAN （n=34） by renal biopsy. The effects of the combined therapy on 24-hour urinary total protein（UTP） and Serum creatinine （SCr） were investigated. Results, The effective rate after treatment of 6 months and 12 months was 80.0% and 83.3% in FSGS group and 82.4% and 85.3% in IgAN group, respectively, with no significant difference between the two groups. The level of UTP decreased significantly after 3 months＇ treatment in IgAN group （P〈0. 05）, while it reduced after 15 months＇ treatment in FSGS group（P〈0.05）, with no significant difference in the level of UTP between the two groups （P〉0.05）, SCr level reduced significantly after 6 months＇ treatment in both groups （P〈0. 05）, with no significant difference between the two groups （P〉0.05）. Conclusion： The combined method of tonifying the kidney, removing dampness and activating blood circulation can improve symptoms and signs, decrease UTP, stabolize renal function.
