功能部位深度烧伤Goulian削痂、刃厚皮片移植的临床观察

摘　　要：目的观察功能部位深度烧伤创面,应用Gou lian削痂及自体刃厚皮片移植术的临床效果。方法对28例共58个功能部位深度烧伤创面,应用Gou lian削痂后:深Ⅱ度创面予以整张或网状自体刃厚皮片移植;Ⅲ度创面予以脱细胞异体真皮加自体刃厚皮片复合移植术。结果削痂过程中无一例损伤肌腱、神经及重要血管,58个功能部位移植物全部成活,术后近、远期随访效果良好关节功能部位无一例发生功能障碍。结论在功能部位深度烧伤削痂植皮术中需要削除坏死组织时,应用Gou-lian削痂、自体刃厚皮片移植不失为一种值得推广的方法。Objective To observe the effect of function part with Goulian tangential excision and skin grafting on the deep burn wound. Methods 28 inpatients with all of 58 deep burn wound on the function part were selected in this study. After the eschar was removed with Goulian, tile wound was covered with meshed skin, autologous split - thickness skin sheet. Results When Goulian tangential excision, bone,nerve and the importance blood vessels have not been injuried. The transplantation was successfully used in 58 deep burn of function part. Conclusion At the deep burn wound of the function part, the necrosis tissue was removed with Goulian and covered with meshed skin. This is an ideal method to reserve the function.
