SPECT in Patients with Diapauses Generalized Seizure Epilepsies

摘  要:本研究对58例全面性发作癫痫患者进行了间歇期SPECT显像,探讨了该类患者脑血流灌注显像的影像特点。静脉注射99Tcm-ECD 30-40m in后进行SPECT显像,以2个以上的断面出现异常放射性药物分布区为阳性病灶。49/58例全面性发作癫痫患者间歇期脑血流灌注显像结果为阳性,阳性检出率为84.5%,其中46例表现为局部脑血流灌注减低。49例阳性检出者中2个以上病灶并存者32例,单个病灶者17例,共检出病灶114个。114个病灶中有52个病灶为某一叶的双侧对称性改变,占45.6%;62个病灶为单侧或双侧非对称性改变,占54.4%。额叶、颞叶、枕叶、顶叶和基底神经核的病灶数分别占50.8%、26.3%、16.7%、5.3%和0.9%。本研究结果显示:本组患者间歇期脑血流灌注显像大多表现为局部脑血流灌注减低,其病灶具有双侧对称性、多灶性或广泛性的特点,且额叶的病灶数明显高于其它各叶。In this study, SPECT perfusion imaging was performed in 58 patients with diapauses generalized seizure epilepsies, and the imaging characteristic was discussed. SPECT perfusion imaging was performed after 30 -40 minutes by mainlined ^99Tc^m - ECD. The positive imaging result was affirmed when an abnormal regions of radiopharmaceutical distribution was found on more than 2 slices. 49/ 58 (84.5%) patients with diapauses generalized seizure epilepsies were positive in SPECT perfusion imaging. Among them, 46 patients images showed local decreased perfusion regions. More than 2 focuses were found in a patient among 32/49 patients, and 1 focus was found in a patient among 17/49 patients. In total, 114 focuses were found in 49 patients. 52/114 (45.6%) focuses were bilateral and symmetry, and 62/114 (54.4%) focuses were not. 50.8% , 26.3% , 16.7% and 5.3% focus distributed on prefrontal, temporal, occipital and apical lobe, respectively. The results showed that local decreased perfusion regions presented in most patients with diapauses generalized seizure epilepsies, and most focuses were bilateral, symmetry and extensive, and the focuses on prefrontal lobe are more than any other lobes.
