摘 要:目的:观察运动处方对超重或肥胖2型糖尿病患者的体质量及血糖、血胰岛素的影响.方法:①选择2002-12/2004-12聊城市人民医院内分泌科超重或肥胖2型糖尿病患者50例,病程1~5年,年龄30~65岁.均对治疗方案知情同意.50例患者被随机分为2组,实验组25例,对照组25例.②实验组在常规药物治疗的同时辅以运动疗法.对照组实施与实验组相同的常规药物治疗.两组干预措施均持续6个月.运动疗法实施方案包括徒步行走、走-跑交替及慢跑、踢毽子舞蹈等方式.③计算体质量指数[体质量(kg)/身高(m)2]和腰臀比.血糖测定采用过氧化物酶法,胰岛素测定采用放射免疫法,糖化血红蛋白采用亲和色谱层析法.上述指标测定在治疗前及治疗结束时完成.④两组间结果比较采用t检验及协方差分析法进行分析.结果:超重或肥胖的糖尿病患者50例均进入结果分析.空腹血糖、糖化血红蛋白、餐后2 h血糖、体质量指数、腰臀比、空腹胰岛素、餐后胰岛素:实验组治疗后明显低于治疗前和对照组治疗后(P<0.05~0.01),空腹胰岛素和餐后胰岛素:对照组治疗后明显低于治疗前[(22.67±3.39),(72.84士7.78)mIU/L;(25.54±5.93),(82.18±9.43)mIU/L,P<0.05,0.01].结论:运动处方可减轻肥胖2型糖尿病患者体质量并改善反映糖尿病病情的相关指标.AIM: To observe the influence of exercise prescription on the body mass, blood glucose and insulin in overweight or obese type 2 diabetic patients, METHODS: ①Fifty overweight or obese type 2 diabetic patients aged 30-65 years with the disease course of 1-5 years were selected from the Department of Endocrinology, Liaocheng People's Hospital between December 2002 and December 2004. They were randomly divided into study group (n=25) and control group (n=25). ②All the patients were given routine drug therapy for 6 months, besides those in the study group received exercise therapy at the same time. The exercise therapy consisted of walking, alteration of walking and running, jogging, dancing with kicking the shuttlecock, etc. ③The body mass index [body mass (kg)/body height (m)^2] and waist-to-hip ratio were calculated. Before and after therapy, blood glucose was detected with peroxidase, insulin with radioimmunoassay, and glycosylated hemoglobin A with affinity chromatography. ④The results were compared and analyzed with the t test and analysis of covariance between the two groups. RESULTS: All the 50 overweight or obese type 2 diabetic patients were involved in the analysis of results. Fasting blood glucose, glycosylated hemoglobin A, 2-hour postprandial glucose, body mass index, waist-to-hip ratio, fasting insulin and postprandial insulin in the study group after therapy were obviously lower than those before therapy and those in the control group after therapy (P 〈 0.05-0.01). fasting insulin and postprandial insulin in the control group after therapy were obviously lower than those before therapy [(22.67±3.39), (72.84±7.78) mIU/L; (25.54 ±5.93), (82.18±9.43) mIU/L, P 〈 0.05, 0.01]. CONCLUSION: Exercise prescription can reduce the body mass and ameliorate the related indexes of diabetes mellitus in obese type2 diabetic patients.
