摘  要:目的:观察巨噬细胞、丙二醛(MDA)及超氧化物歧化酶(SOD)在大鼠心梗后早期左室重构过程中的变化及当归的治疗作用,探讨干预机制。方法:结扎SD大鼠冠脉前降支制备心梗模型,随机分为假手术组(sham组),心梗组(MI组)和心梗当归组(MI+Angelica组)。术后24h开始给药,MI+Angelica组腹腔注射当归2mL/(100g.d),sham组和MI组腹腔注射等量生理盐水。于术后1W、2W、4W记录左室血流动力学变化,测定血浆中MDA和SOD浓度改变,并检测非梗死区心肌胶原及巨噬细胞的变化(免疫组化法)。结果:①与sham组相比,MI组和MI+Angelica组非梗死区心肌巨噬细胞阳性表达率显著升高(P<0.01),以1W的阳性表达率为最高,但MI+Angelica组低于MI组(P<0.01);②与sham组相比,MI组和MI+Angelica组术后各时间点血浆中MDA浓度升高,SOD浓度降低(P<0.01),但MI+Angelica组MDA低于MI组,SOD高于MI组,4W时有统计学意义(P<0.05);③术后各时间点MI组心功能受损,于术后4W心肌胶原容积分数(CVF)及血管周围胶原面积(PVCA)明显升高(P<0.01);与MI组比较,术后4W MI+Angelica组心功能改善,CVF及PVCA降低(P<0.05)。结论:当归可能通过清除体内自由基、增强机体的抗氧化能力、抑制巨噬细胞在非梗死区的浸润和聚积,阻断促左室肥厚和反应性纤维化发生等环节,防治和逆转左室重构。Aim: :To investigate ①the role of macrophage infiltration and malondialdehyde (MDA) and superoxide dismutase (SOD) concentration during the development of early left ventricular (LV) remodeling in rats after myocardial infarction (MI); ②the cardioprotective effect of Angelica on LV remodeling post - MI; and ③ mechanisms of LV remodeling post- MI and the inhibitory effect of Angelica. Methods: Sprague -Dawley(SD) rats were subjected to MI by ligating the left anterior descending coronary artery or sham operation and randomly divided into three groups: sham, MI and MI + Angelica. After 24 hours of ligation, rats received Angelica (2 mL/100(g·d) ,i.p. ) (MI + Angelica group) or saline (sham and MI groups). LV hemodynamics were measured and plasma samples were collected for determination of MDA and SOD concentration, and rats in the sham, MI and MI + Angelica groups were killed at week 1, week 2 and week 4, respectively. Histological and immunohistochemical studies were performed on the heart to examine interstitial and perivascular collagen, macrophage infiltration in the non- infarcted area. Resuhs:①In the MI group, macrophage number and in the non- infarcted myocardium were significantly upregulated compared to sham at week 1 post - MI and remained elevated at week 4 ( P 〈 0.01). Angelica significantly decreased macrophage infitration at selected time points ( P 〈 0.01 vs. MI); ②Compared with the sham group, the plasma MDA concentration was increased, while the plasma SOD concentration was decreased in the MI group ( P 〈 0.01), Angelica decreased MDA and increased SOD concentration respectively at week 4 ( P 〈 0.05); ③collagen volume fraction (CVF) and perivascular collagen area (PVCA) were increased significantly in the MI group compared to sham at week 4 post - MI ( P 〈 0.01 ), whereas both were decreased in the MI + Angelica group ( P 〈 0.05 vs. MI) ; and ④ cardiac function was markedly decrea
