2型糖尿病伴高血压患者胰岛素抵抗与血压关系的探讨

摘　　要：目的观察2型糖尿病伴高血压患者的胰岛素敏感性,以探讨胰岛素抵抗与糖尿病伴高血压间的关系。方法对60例2型糖尿病合并高血压患者与23例未合并高血压患者的体重指数、血压、空腹血浆葡萄糖、免疫反应性胰岛素及真胰岛素、胰岛素敏感性和胰岛β细胞功能进行比较。结果糖尿病伴和不伴高血压组间除血压显著改变外,真胰岛素水平及其胰岛素抵抗程度相似;且无论体重指数小于、大于、等于25 kg/m2,亦显示血压差异有显著性意义。结论胰岛β细胞功能随病程发展逐步衰减,可能是在自然病程晚期胰岛素水平及胰岛素抵抗程度与血压“相关显著性消失”的主要原因。Objective To probe into the relationship between insulin resistance（IR） and diabetes accompanied with high blood pressure by observing the insulin sensitivity of patients suffering from type 2 diabetes mellitus （T2DM） accompanied with high blood pressure. Methods Comparing the body mass index （ BMI）, blood pressure, fasting plasma glucose, and the function of 13 cells of 60 T2DM cases having high blood pressure with those of 23 T2DM cases without high blood pressure. Results The two groups differ greatly in blood pressure,but with similar trueinsulin（TI） and IR;there is great difference in blood pressure whether their body mass index is less than, more than or equal to 25kg/m^2. Conclusions The loss of obvious relationship between insulin level and insulin resistance is mainly due to the fact that the function of 13 cells decreases gradually with the development of the disease at the advanced stage.
