二次脑创伤对颅脑伤预后的影响

摘　　要：目的探讨高热、低血压、低氧血症、电解质紊乱、颅内压和脑灌注压异常等二次脑创伤指标对颅脑伤预后的影响。方法对我院近4年来收治中型以上颅脑伤122例病人资料进行回顾性总结分析。结果高热、低血压、低氧血症、电解质紊乱、颅内压与脑灌注压异常严重影响颅脑伤患者的预后,显著增加患者的重残率、植物生存率及病死率(P<0.01)。结论高热、低血压、低氧血症、电解质紊乱、颅内压与脑灌注压异常均属于二次脑创伤指标,都直接参与了颅脑伤的继发性病理损害。高度重视监测和严格控制这些二次脑创伤指标的变化,对提高颅脑伤救治水平有重要意义。Objective To explore the effects of secondary insults on the prognosis of primary injury.Methods The data of 122 cases with brain trauma were analyzed retrospectively. Results The prognosis of patients was strongly influenced by hyperpyrexia, hypotension , hypoxemia , electrolyte disturbance , intracranial pressure （ICP）and cerebral perfusion pressure（CPP） abnormity . They increased significantly the severe disability rate, vegetative state rate and mortality rate（ P〈 0.01）. Conclusion Hyperpyrexia, hypotension , hypoxemia , electrolyte disturbance , intracranial pressure （ ICP）and cerebral perfusion pressure（CPP） abnormity are all the indexes of secondary brain insults. It is very important to monitor and control the changes of these secondary brain insult indexes for improvement of brain trauma treatment.
