摘  要:目的观察重组人白细胞介素1受体拮抗剂(rhIL1ra)对窒息后肾损伤的保护作用及其机制。方法检测对照组及rhIL1ra(+rhIL1ra组)或生理盐水(+NS组)处理新生鼠窒息后复氧2、24和48h肾组织炎症反应及肾损伤指标的变化。结果与对照组(13只)比较,窒息后复氧2h(10只)、24h(11只)、48h(10只)+NS组的白细胞数(WBC)、白细胞介素1(IL1)、IL8、IL6、一氧化氮(NO)、内皮素1(ET1)、左肾系数(LRC)、肾小管损伤评分均升高,差异均有显著性(P<0.05或P<0.01)。与窒息后复氧+NS组比较,窒息后复氧2h(10只)、24h(10只)、48h(11只)+rhIL1ra组上述指标除IL6外均降低,差异也均有显著性(P<0.05或P<0.01);而窒息后复氧24h、48h+rhIL1ra组IL6均降低,差异均有显著性(P<0.05或P<0.01)。结论rhIL1ra对窒息后肾损伤具有保护作用,其机制可能与抑制窒息后肾组织炎症反应有关。Objective To observe the role of recombinant human interleukin - 1 receptor angonist (rhIL -1ra) in protecting kidney from injury induced by asphyxia in neonatal rats. Methods Neonatal rats were used as experimental animals. The changes in intrarenal inflammatory response and renal injury were examined in the control group (n= 13), and 2, 24 and 48 hours after asphyxia followed by normal saline treatment in those treated witti rhIL -1ra. Results In normal saline group, the white blood cell count, the blood interleulein - 1 (IL - 1), IL - 8, IL - 6, nitric oxide(NO), endothelin (ET - 1) levels, and the renal coefficient (LRC), the scores of injuried tubules of the left kidney were significantly increased at 2 hours (n=10), 24 hours (n=11), and 48 hours (n=10, P〈0. 05 or P〈0. 01). Compared with the normal saline group, the levels of the above parameters, except IL - 6, were significantly decreased in rhIL -1ra treatment group at the same time points (P〈0. 05 or P〈0. 01). Serum IL - 6 at 24 hours and 48 hours was also decreased in rhIL -1ra treatment group significantly (P 〈 0. 05 or P (0. 01). Conclusion The results suggest that rhIL -1ra may protect renal injury after asphyxia via inhibiting intrarenal inflammatory response.
