摘  要:目的:探讨核因子-κB在角膜移植排斥反应中的作用,为进一步研究角膜移植排斥反应发病机制及治疗提供新的思路。方法:实验于2004-12/2005-02在南方医科大学珠江医院眼科完成。实验分组:同基因移植组Wistar大鼠5只为供者,Wistar大鼠10只为受者;同种异体移植组Wistar大鼠5只为供者,SD大鼠10只为受者。同种异体移植+环孢素A治疗组Wistar大鼠5只为供者,SD大鼠10只为受者。建立大鼠穿透性角膜移植动物模型,用免疫组织化学染色法检测角膜移植术后植片中核因子-κB和细胞间黏附分子-1表达,显微镜下记数植片中央区400倍视野中阳性细胞数的平均值,并以植片排斥反应指数及病理学表现作参照。结果:纳入实验大鼠30只,均进入结果分析。①同基因移植组角膜植片中的角膜上皮细胞、基质细胞及内皮细胞有核因子-κB和细胞间黏附分子-1的微弱表达,表达强度分别为3.16±1.25,2.83±1.54;同种异体移植组角膜上皮细胞、内皮细胞、基质层细胞及新生血管内皮细胞核因子-κB和细胞间黏附分子-1表达增强,分别为16.77±2.76,13.63±1.93;同种异体移植环孢素A治疗组角膜上皮细胞、内皮细胞、基质层细胞及新生血管内皮细胞有核因子-κB和细胞间黏附分子-1的弱表达,分别为6.77±1.86,4.57±1.91。②各组间核因子-κB和细胞间黏附分子-1的表达强度有显著性差异(P<0.01)。③对比观察发现,各组角膜植片中NF-κB和细胞间黏附分子-1的表达部位和强度相似,相关性分析核因子-κB与细胞间黏附分子-1的表达强度呈正相关(r=0.875,P<0.01),且核因子-κB的表达与排斥反应指数也具有明显的正相关性(r=0.829,P<0.01)。结论:核因子-κB可能通过调控细胞间黏附分子-1等基因的表达,参与角膜移植排斥反应的发生,在角膜移植免疫中起着中心调控者的作用。而免疫抑制剂CsA通过减弱核因子-κB核转位和发挥活性,抑制排�AIM: To investigate the role of nuclear factor-kappa B (NF-κB) in corneal graft rejective reaction, so as to provide new thought for the further research of the pathogenesis and treatment of corneal graft rejective reaction. METHODS: The experiment was carried out in the Department of Ophthalmology, Zhujiang Hospital of Southern Medical University between December 2004 and February 2005. Grouping: homogenic transplantation group (5 Wistar rats as donors and 10 Wistar rats as recipients), allograft group (5 Wistar rats as donors and 10 SD rats as recipients) and allograft + ciclosperin A treated group (5 Wistar rats as donors and 10 SD rats as recipients). Rat models of penetrating orthotopical corneal transplantation were established, the expressions of NF-κB and intracellular adhesion molecule-1 (ICAM-1) after the corneal transplantation were detected with the immunohistochemical staining, and the average number of positive cells was enumerated in corneal central zone (×400 micro, field). The rejection index and pathological manifestations were taken as controls.RESULTS: All the 30 rats were involved in the analysis of results. ① NF-κB and ICAM-1 weakly expressed on the corneal endothelial cells, keratocytes and corneal endothelial cells in the homogemc transplantation group, the expressed intensities were 3.16±1.25 and 2.83±1.54 respectively; NF-κB and ICAM-1 obviously expressed on corneal epithelial cells, keratoeytes, corneal and new vascular endothelial cells in the allograft group, which were 16.77±2.76 and 13.63±1.93 respectively; NF-κB and ICAM-1 expressed weakly on corneal epithelial cells, keratocytes, corneal and new vascular endothelial cells in the allograft + ciclosporin A treated group, which were 6.77±1.86 and 4.57±1.91 respectively. ② The expressed intensities of NF-κB and ICAM-1 were significantly different among the groups (P 〈 0.01). ③ The comparative observation showed that the expressed site and intensity of NF-κB and ICAM-1 were
