摘 要:背景:肢体负压治疗有扩张肢体血管,改善末梢微循环的效应。P物质具有强大的血管扩张活性,其参与皮肤伤害性刺激的感受与局部血管功能的调节。目的:观察肢体负压对周围动脉闭塞性病变犬皮肤中P物质免疫反应阳性神经纤维的影响。设计:随机对照的实验。单位:解放军第四军医大学西京医院普外三科。材料:实验于2003-04/2004-05在第四军医大学西京医院动物实验室完成。健康杂种犬17只,随机分为3组,治疗组10只,非治疗组5只,正常对照组2只。干预:患肢负压治疗:动物浅麻醉后,将左后肢置入自制负压舱中,进行负压治疗,压力为-12kPa,持续15min,1次/d,连续10d。①治疗组:将动物制作左后肢缺血模型,14d后开始行患肢负压治疗。10d结束后,行动物灌注,分别切取患肢第2足趾皮肤、L1~5的脊髓及背根神经节,行免疫组织化学染色,检测前列腺素E1免疫反应阳性神经纤维。②非治疗组:除不进行患肢负压治疗外,处理、检查均同治疗组。③正常对照组:不行缺血模型制作及负压治疗,仅行免疫组织化学染色检测。主要观察指标:各组犬皮肤P物质免疫阳性神经纤维变化。结果:17只犬均进入结果分析。各组犬皮肤P物质免疫阳性神经纤维变化:治疗组在真皮结缔组织中及层血管旁P物质免疫反应阳性神经纤维均较非治疗组减少[(24.70±4.6),(43.49±6.3)μm/mm2,P<0.01],但仍较正常对照组增多[(18.10±5.4)μm/mm2,P<0.01];非治疗组真皮层中P物质免疫反应阳性神经纤维较正常增多(P<0.01),在真皮结缔组织中和小血管旁P物质阳性神经纤维明显增加,染色加深。正常对照组趾皮的角质层中,未见有P物质免疫反应阳性纤维。在真皮层中,可见有P物质免疫反应阳性神经纤维。结论:提示肢体负压疗法可促进皮肤感觉神经纤维中P物质的释放。BACKGROUND: Limb negative pressure treatment can dilate limb vessels and improve terminal microcirculation. P-substance has strong vasodilative activity and is involved in the sensation of the skin to traumatic stimulation and the modulation of local vascular function. OBJECTIVE: To observe the influence of limb negative pressure on cutaneous P-substance immunoreactive nerve fibers in dogs with peripheral arterial occlusive disease. DESIGN: Randomized controlled experiment. SETTING: The 3^rd Department of General Surgery, Xijing Hospital Affiliated to the Fourth Military Medical University of Chinese PLA. MATERIALS: This experiment was conducted at the Animal Laboratory of Xijing Hospital Affiliated to the Fourth Military Medical University of Chinese PLA between April 2003 and May 2004. Totally 17 healthy hybrid dogs were randomized into 3 groups, namely, treatment group of 10 dogs, non-treatment group of 5 dogs, and normal control group of 2 dogs. INTERVENTIONS: Negative pressure treatment on affected limbs: After superficial anesthesia, the left hindlimbs of the animals were put into the home-made negative cabin for negative pressure treatment with pressure designed as -12kPa, for 15 minutes, once a day for consecutive 10 days. ① Treatment group: The left hindlimb ischemic model was prepared 14 days before starting 10-day negative pressure treatment; after that the animals were subjected to infusion, the skin of the 2^nd toe of affected limbs, as well as L1-5 spinal cords and dorsal root ganglion were obtained for immunohistochemical (IHC) staining. Meanwhile prostaglandin E1 immunoreactive nerve fibers were detected.② Non-treatment group: The animals received the same treatment and examination as treatment group except for negative pressure. ③ Normal control group: No ischemie model was prepared or negative pressure treatment was given except for IHC staining. OUTCOME MEASURES: Changes of cutaneous P-substance immunoreactive nerve fiber in each group. RESULTS: Totally 17 dog
