随机双盲安慰剂对照评价米诺地尔治疗女性雄激素性秃发

摘　　要：用米诺地尔治疗男性雄激素性秃发已取得了较好的疗效，为进一步研究米诺地尔治疗秃发的疗效，进行了外用３％米诺地尔治疗女性雄激素性秃发的研究。方法：采用随机双盲安慰剂对照前瞻性研究。以医师与患者共同观察，毛发计数和拍照作为疗效判断方法。３％米诺地尔生发水或安慰剂２ｍｌ外用，日２次。治疗４个月后，安慰剂组改用米诺地尔。病例：选择年龄＜６０岁、病程＜２０年的女性雄激素性秃发患者。治疗组２４例，对照组２１例。结果：观察治疗４个月，治疗组、对照组有效率分别为５８．３％（１４／２４）和１４．２％（３／２１），Ｐ＝０．００２３；治疗１２个月两组有效率分别为７２．２％（１３／１８）和７３．３％（１１／１５）。毛发计数，治疗组治疗４个月非毳毛计数６０％病例超过治疗前水平，治疗１２个月６５％病例超过治疗前水平，３２％病例超过治疗４个月水平。治疗１～２个月毳毛均数上升，治疗３～４个月毳毛向非毳毛转变，毳毛数减少；治疗２个月非毳毛均数开始上升，并连续上升８个月后平缓不再上升；治疗４个月时治疗组与对照组非毳毛计数分别为２０．７±６７．０和１０．６±６４．８根（Ｐ＝０．０３０）。部分病例治疗前后拍照也显示较为理想。虽然少数病例被判定为治疗?Wehaveobtainedsatisfactoryresultinthetreatmentofmalepaternbaldnesswithtopi-calminoxidilpreparation.Forfurtherstudyingweperformedthestudyoffemaleandrogeneticalopeciatreatedtopicalywith3%minoxidil.Methods:Randomizeddouble-blindplacebocontroledprospectivestudywasused.Eficacywasdeterminedbydoctor'sandpatients'observation,haircountandtakingpicture.Either2mlof3%minoxidilsolutionorplacebowasusedtwicedaily.Subjectswhobeganwithplacebowerecrosedoverto3%minoxidilafterthefirstfourmonths,andbothgroupscontinuedtouseminoxidilforanadditional8monthsinopentrial.Patients:Patientswithfemaleandrogeneticalopeciaage<60yearsold,durationofdisease<20years,andlocalresidentsenteredintothestudy.Therewere24casesinthetreatmentgroupand21inthecontrolgroup.Results:Doctor'sandpatients'observationshowedthattheratesofefectivenessofbothminoxidilandcontrolgroupswere58.3%(14/24)and14.2%(3/21)respectively(P=0.0023),afterfourmonthtreatment;72.2%(13/18)and73.3%(11/15)aftertwelvemonths.60%casesinthetreatmentgroupthenonvelushairsatfourmonthsoutnumberedthoseatbaseline,65%casesattwelvemonths,andthehaircountin32%casesattwelvemonthswasgreaterthanthatatfourmonths.Themeanvelushaircountofminoxidilgroupincreasedafter1～2monthtreatment,velushairsappearedtochangeintononvelushairsafter3～4monthsandvelushairsbegantodecrease.Nonvelushairsbegantoincreaseaftertwomonthsandcontinuedtoincreaseforeightmonths.Meannonvelushairsattheendoffourmonthswere20.7±67.0inminoxidilgroupand10.6±64.8incontrolgroup,thediferenceissignificant(P=0.03).Thephotographsofsomepatientstakenbeforeandaftertreatmentalsoshowedsatistactoryresult.Al-thoughsomeofthepatientsareofnoefect,theyarewilingtousethedrugforalongperiodforthecontrolofthedevelopmentofalopecia.
