摘 要:目的:观察自体微小颗粒骨复合牛骨形态发生蛋白的成骨效果。方法:实验于2002-10/2003-05在哈尔滨医科大学附属第二医院动物实验中心完成。将30只新西兰白兔两侧桡骨干中段造成1.5cm长骨缺损模型,随机分为实验组21只,空白对照组9只。其中实验组左侧植入颗粒骨+骨形态发生蛋白,右侧植入颗粒骨;空白对照组双侧植入明胶海绵。分别于术后2,4,12周实验组各取7只、空白对照组各取3只对兔桡骨植入物进行大体形态、放射学检查及组织学检查,12周进行生物力学试验。结果:纳入兔30只,均进入结果分析。①兔桡骨植入物大体、X射线与组织学检查结果:术后12周实验组两种方法均可以修复节段性骨缺损,但左侧无论从成骨时间及成骨效果上都要优于右侧,空白对照组无骨愈合现象。②兔桡骨植入物生物力学测试结果:证明实验组左侧在最大应力方面要优于右侧[(101.03±12.49),(73.71±9.75)N,P<0.05];在弹性模量上二者无显著性差异。结论:自体微小颗粒骨可较好的修复骨缺损,但复合骨形态发生蛋白后在成骨时间和成骨质量上更优。AIM: To observe the effect of the composite of autologous morsellized cancellous bone grafts and bone morphogenetic protein (BMP) in bone formation.METHODS: The experiment was carried out in the animal experimental center of the Second Affiliated Hospital of Harbin Medical University between October 2002 and May 2003. Thirty New Zealand white rabbits were made into models of bilateral 1.5cm radius bone defect, and then randomly divided into study group (n=21) and blank control group (n=9). In the study group, a composite of morsellized cancellous bone grafts and BMP was placed into the bone defect of left radius, and morsellized cancellous bone grafts was placed into the right one; In the blank control group, the defect was filled only with gelatin sponge for both sides. Seven rabbits in the study group and 3 rabbits in the blank control group were selected at 2, 4 and 12 weeks after operation respectively to be examined physically, radiographically, histogically and biomechanically. RESULTS: All the 30 rabbits were involved in the analysis of results. ① The results of the physical, histogical and radiographical examinations showed that at 12 weeks, both of methods in the study group could repair the segmental defect of bone, but the efficacies of the speed and effect of bone formation on the left were superior to those on the right. No bone healing was observed in the blank control group. ② The results of the biomechanieal test of radius implant: It was proved that the biggest stress on the left was better than that on the right in the study group [(101.03± 12.49), (73.71±9.75) N, P 〈 0.05], but there was insignificant difference in the modulus of elasticity. CONCLUSION: The autologous morsellized cancellous bone grafts can be used effectively to repair bone defect, but the speed and effect of bone formation for autogeneous morselized cancelous bone is better than BMP.
