摘 要:目的:比较智能方向性技术与全方向性技术对听力障碍患者噪声环境下言语识别率的影响,探讨智能方向性技术对噪声环境下言语识别率的改善作用。方法:选择2004-06/11在斯达克(中国)有限公司听力技术中心成功验配助听器的15例听力障碍患者。将配戴者助听器的记忆程序设定为:记忆1,智能方向性技术模式,记忆2,全方向性技术模式及记忆3,电话模式。使用记忆1,配戴者在噪声环境下感受智能方向性技术的效果;分别模拟现实生活安静、噪音环境下使用记忆1和记忆2,记录并分析患者应用不同的方向性技术在不同环境下的言语识别率、聆听满意度及偏好记忆。结果:按实际处理分析,15例患者均进入结果分析。①言语识别率:在噪声环境下应用智能方向性技术模式较应用全方向性技术模式时提高[(78±8)%,(60±7)%,P<0.05]。②聆听满意度:在噪声环境下应用智能方向性技术模式较应用全方向性技术模式时提高(86%,33%,P<0.05)。③偏好记忆:15例患者(15只助听器)在安静和噪声环境下分别偏好全方向性技术和智能方向性技术模式。结论:听力障碍者在噪声环境下偏好智能方向性技术模式,应用智能方向性技术模式时的言语识别率和满意度均较应用全方向性技术模式时提高。听力障碍患者应用智能方向性技术有利于提高噪声环境下的聆听效果。AIM:To compare the effects of precision directional imaging(PDI)and omni-directional imaging (ODI)on speech discrimination score in patients with dysaudia under noise environment, and explore the ameliorative effects of PDI on the speech discrimination score under noise environment. METHODS: Fifteen patients with dysaudia, who worn successfully a hearing aid at the Listening Technology Center, Starkey Laboratory, Inc. China Branch between June and November 2004, were selected. The memory process of wearing hearing aid was set: memory 1, PDI pattern; memory 2, ODI pattern and memory 3, telephone pattern. Using memory 1, the wearer felt the effects of PDI under noise environment, and modeling the actual quiet living, and the noise living; Using memory 1 and 2, the speech discrimination score, listening satisfactory degree and favorable memory were recorded and analyzed with different directional imaging under different environment. RESULTS: According to the intention-to-treat analysis, 15 patients were all involved in the result analysis. ① Speech discrimination score: It was higher with the PDI pattern than that with the ODI pattern under noise environment [(78±8)%, (60±7)%, P 〈 0.05]. ② Listening satisfactory degree: It was enhanced with the PDI pattern than that with the ODI pattern under noise environment (86%,33%,P 〈 0.05). ③ Favorable memory: Fifteen patients (15 hearing aids) liked the ODI pattern and PDI pattern under quiet and noise environment, respectively. CONCLUSION: The patients with dysaudia favor the PDI pattern under noise environment. The speech discrimination score and satisfactory degree when using the PDI pattern enhance compared with the ODI pattern. The application of PDI in patients with dysaudia is helpful for the hearing effect under noise environment.
