母婴血瘦素与胎儿发育的相关研究

摘　　要：目的:探讨足月新生儿脐血及其母亲静脉血瘦素水平与新生儿生长因子及生长参数的关系。方法:采用免疫放射法测定了43例足月新生儿脐血及其母亲分娩时周围静脉血的血清瘦素水平,并同时测定新生儿脐血胰岛素(INS),胰岛素样生长因子-1(IGF-1),胰岛素样生长因子结合蛋白-3(IGFBP-3),生长激素(GH)。采用SPSS10.0forWindows进行相关分析。结果:(1)新生儿脐血瘦素水平为(13.40±4.77)μg/L,明显低于母血瘦素水平(35.04±24.22)μg/L,但无相关性(r=0.071,P>0.05)。(2)新生儿脐血、母血的瘦素水平与脐血INS、IGF-1、IGFBP-3、GH之间无相关性。(3)新生儿脐血的瘦素水平与其出生体重呈正相关(r=0.320,P=0.05);母血瘦素水平与其新生儿出生体重无相关(P>0.05),与新生儿身长呈负相关(r=-0.342,P=0.05)。(4)新生儿脐血IGF-1、IGFBP-3与新生儿出生体重、身长、足长、胎盘重量均呈显著正相关(P<0.01);GH、INS与4个生长参数之间无相关性(P>0.05)。结论:结果提示新生儿瘦素来源于自身,且参与了胎儿体重的调节,而母亲瘦素水平不影响胎儿体重。IGF-1、IGFBP-3与新生儿发育参数呈显著正相关,而与脐血瘦素水平无相关,说明胎儿发育受多种因素影响,生长因子与瘦素在胎儿发育中均起重要作用。Objective： To explore the relationships between leptin levels in maternal blood,umbilical blood and growth factors ,growth parameters of newborn. Methods： Levels of leptin in cord blood from 43 full-term neonates and venous blood of their mothers at delivery were measured by immuno radiometric assay （IRA）; by the way, cord blood Insulin （INS）, growth hormone （GH）, Insulin-like growth factor-1 （IGF- 1）, Insulin-like growth factor binding protein-3 （IGFBP-3） were determined by RIA. The date were ana- lyzed by SPSS 10.0 for windows. Results： （1） Mean leptin level in cord blood was （13.40±4.77） ng/ml significantly lower than that in maternal sera（35.04±24.22） ng/ml, but no correlation was found （r = 0.071, P 〉 0.05）. （2） Leptin levels of maternal and cord blood did not show correlations with umbilical blood INS,GH, IGF-1 and IGFBP-3. （3） The cord blood leptin levels correlated with birth weight （r = 0.320, P = 0.05）, but no correlation was found between maternal leptin concentrations and birth weight. （4） Cord blood IGF-1, IGFBP-3 correlated positively with birth weight,birth length ,foot length and placental weight （P 〈 0.01）. There was no correlation between INS,GH and four growth parameters（P 〉 0.05）. Conclusion： Leptin levels were lower in cord blood than in maternal sera but there was no correlation between leptin levels of mothers and newborns. The cord blood leptin levels were correlated with birth weight. The result shows that neonatal leptin may come from themselves,and plays an important role in regulation of fetal weight. There was significantly positive correlation between cord blood IGF-1、IGFBP-3 and newborn growth parameters,but no correlation with cord blood leptin. It is suggest that fetal growth process is governed by multiple factors; growth factors and leptin both play an important role in the fetal growth.
