摘　　要：目的探讨胰岛素样生长因子-Ⅱ(IGF-Ⅱ)和肾上腺髓质素(ADM)在急性链球菌感染后肾小球肾炎(APSGN)中的变化及临床意义。方法应用放射免疫法测定36例APSGN患儿(观察组)入院和住院第15 d时血清IGF-Ⅱ与ADM水平,并与正常儿童(对照组)进行比较。结果入院时APSGN患儿与正常儿的血清IGF-Ⅱ、ADM的含量间差别均有显著性意义(P<0.05);入院第15 d时APSGN患儿与正常儿血清IGF-Ⅱ含量间差别仍有显著性意义(P<0.05),而血清ADM含量间差别无显著性意义(P>0.05)。结论APSGN患儿存在着IGF-Ⅱ和ADM水平改变。ADM增高可能与APSGN急性期患儿因机体防御作用使血压升高有关;IGF-Ⅱ降低可能与APSGN患儿肾功能受损和免疫功能失调有关,但其在APSGN发病中的作用值得进一步研究探讨。Objective To explore the changes and the clinical significance of serum levels of insulin - like growth factor- Ⅱ （IGF- Ⅱ ） and adrenomedullin （ADM） in children with acute poststreptococcal glomerulonephritis （APSGN）. Methods Serum levels of IGF - Ⅱ and ADM in 36 cases with APSGN （ observation group） were measured with radioimmunoas- say on the first and the fifteenth day of admission. The results were contrasted with 30 healthy children （ control group）. Results On admission, the IGF - Ⅱ level in the observation group was much lower than that in the control group while the ADM level was higher than that in the control group （ P 〈0.05）. On the fifteenth day of admission, the IGF - Ⅱ level remained lower （ P 〈 0. 05 ） and there was no difference among serum ADM contents. Conclusion There are changes in serum levels of IGF - Ⅱ and ADM in children with APSGN. The increase of the ADM level is associated with increased blood pressure and defense mechanisms in acute phase. The decrease of the IGF - Ⅱ level is associated with renal function injury and immunologic function misbalance. But their roles in APSGN still need further investigation.
