小腿滑膜炎的超声诊断

摘　　要：目的探讨高频超声检查小腿滑膜炎的价值。方法回顾性分析25例滑膜炎的超声资料,根据肿块的位置、大小、形态、内部回声进行诊断,根据部位及有无交通性作出分型。17例患者经穿刺细胞学检查证实。结果4例B超表现为椭圆形无回声区,16例梭形无回声区,5例为低回声区,边界清楚,后方回声增强,1例患者半膜肌腱处可见鹅足样囊肿;其中4例为窝囊肿,21例为滑膜炎,1例合并髌上囊滑膜炎。结论B超能准确诊断滑膜病变,并进行定位诊断,对选择手术径路有重要意义。Objective To investigate the value of high frequency ultrasound in detecting synovitis of the calves. Methods of 25 patients were analyzed retrospeetively, aeeording to the position, size, shape, the inner echoes of synovits were diagnosed. Different locational calves cyst was classified aecording to synovitis position and whether the eyst was eommnnieating with the knee joint. 17 eases were verified by aspiration. Results 4 eases of oval mass were revealed as eeho - free areas, 16 eases of spindle eeho- free areas and remaining 5 hypoecho genie area with well - defined margin and posterior aeonstie enhaneement sonographieally, lpatient showed bursa anserina in the semimembranosus tendon; ultrasonic diagnosis bad been made for 25 cases patients included 4 eases popliteal eysts, 21eases synovits, 1 cases eomplieated prepatellar bursitis. Conclusion The results demonstrate that ultrasound examination ean obtain aeeurate diagnosis for synovial diseases of the ealves. Moreover, it is of important sing nifieanee for locate the position of the eyst for aspiration and for therapeutie planning.
