摘  要:目的探讨产前应用地塞米松对早产儿颅内出血的预防效果.方法将住院待产有早产可能的孕妇随机分成2组,观察组于入院后给予地塞米松静脉点滴,10mg/天,连用1~2天,对照组无干预措施.新生儿出生后常规作头颅超声检查了解颅内出血情况.结果在胎龄小于35周的小胎龄早产儿,观察组颅内出血发生率为52.0%,显著低于对照组的65.2%(x2=4.739,P=0.029);而在胎龄满35周以上的较大胎龄的早产儿,两组颅内出血发生率无显著差异(x2=0.111,P=0.740).结论产前应用地塞米松对小胎龄早产儿颅内出血有良好的预防效果.Objective To investigate the clinical value of maternal prenatal administration of dexamethasone for decreasing incidence of intracranial hemorrhage of preterm infants. Methods 287 cases of pregnant women which have likelihood of premature delivery were divided into two groups, The pregnant women in study group was given 10 mg/d of dexamethasone every day for one or two days, and those in control group was not given dexamethasone. The transcranial ultrasonography was performed to learn if the neonates suffered from intracranial hemorrhage. Results The incidence of intracranial hemorrhage was 52.0 % in study group and 65.2 % in control group respectively in infants whose gestational age were less than 35 weeks( X^2 = 4. 739, P=0. 029) , while in the incidence of intraeranial hemorrhage, there was no significant difference between the two groups for infants whose gestational age were more than 35 weeks( X^2 = 0.111, P=0. 740). Conclusion Prenatal maternal administration of dexamethasone has a good preventing effect for intracranial hemorrhage of preterm infants whose gestational age were less than 35 weeks.
