丹参粉针联合普康喜治疗糖尿病肾病的临床观察

摘　　要：目的:观察丹参粉针联合普康喜(前列腺素E1)治疗糖尿病肾病(DN)的临床疗效。方法:72例DN患者随机分为2组,治疗组40例,其中早期糖尿病肾病(IDN)19例,临床期糖尿病肾病(CDN)21例,采用丹参粉针联合普康喜进行治疗;对照组32例,其中IDN15例,CDN17例,进行糖尿病的常规治疗。测定2组24h尿蛋白排泄率(24hUAER)、血浆粘度、红细胞聚集指数、总胆固醇(TC)、甘油三酯(TG)、尿素氮(BUN),并对2组各指标进行了比较。结果:(1)治疗组IDN、CDN患者治疗后24hUAER均较治疗前明显下降(P<0.05-0.01),亦明显低于对照组(P<0.05-0.01);(2)治疗组IDN、CDN患者治疗后血浆粘度、红细胞聚集指数、BUN均较治疗前明显下降(P<0.05-0.01),亦明显低于对照组(P<0.05-0.01);TC、TG均较治疗前有所下降,但差异无统计学意义(P>0.05)。结论:丹参粉针联合普康喜治疗DN能减少尿白蛋白的排除,且能降低血浆粘度、红细胞聚集指数、BUN,对肾功能有一定的保护作用,并能延缓病情发展,疗效确切而且安全。Objective： To study the clinical effect of combination of danshen and alprostadil（prostaglandin E1） to treat patients with diabetic nephropathy（DN）. Methods： Seventy-two patients with DN were randomly divided into two groups： test group, using the drugs of danshen and alprostadil with routine antidia- betics, that was consisted of 40 patients, including 19 cases of initial DN and 21 cases of clinical DN; control group with routine antidiabetic only was consisted of 32 patients, initial and clinical DN patients were 15 and 17 respectively; the observed indicators were 24 h urine albumin eliminate rate, blood viscosity, erythrocyte collect index, total cholesterol, triglycerides, blood urea nitrogen and comparing the indicators change in two groups. Results： （1） After treatment with danshen and alprostadil, the level of 24 h urine albumin eliminate rate had evident descent than that of before in initial and clinical DN patients （P 〈0.05 -0.01）, （2） Collect index, blood urea nitrogen had evident descent also than before treatment in test group and the mentioned above the level of control patients were higher than test group （P 〈0.05-0.01）. In test group, the level of triglycerides and total cholesterol had some descent, but the difference had not statistic significance. Conclusions： The treated measure of combination danshen with alprostadil can descend the removal of urine albumin, and descend the level of blood viscosity, erythrocyte collect index, blood urea nitrogen in patients with DN and protect their kidney function and delay the disease development; this treated measure is safe and the effect is definite.
