四川省藏区某县肺结核病人发现水平及其影响因素分析

摘　　要：目的分析四川省藏区若尔盖县及其不同区的肺结核病人发现水平,并讨论影响病人发现的相关因素。方法利用初诊病人登记表和初诊病人问卷调查表,对获取的信息运用聚类分析、方差分析和χ2检验等统计学方法进行分析。结果2003年7月7日至2004年7月6日若尔盖县结核病初诊病人就诊率7.10‰,肺结核病登记率220.7/10万,以县城为中心的区(多玛区)和半农半牧区(铁布区、巴西区)的病人发现水平高于牧区(唐克区、纳摩区)。铁布区和多玛区初诊病人的首诊医生为结防医生的比例高于其他区,分别为16.7%和22.6%,并且铁布区的初诊病人对结核病的了解情况好于其他各区。结论政府重视、广泛的健康教育和提高卫生服务的地理可及性是提高结核病人发现水平的关键。Objective To analyse the levels of TB case-finding and influence factors in Ruoergai county of Sichuan province. Methods The involved information was obtained from the registration and the questionnaire of first visit patient, statistical methods such as cluster analysis, chi-square test and analysis of variance were used. Results The rate of seeking medical care was 7.10%o, and the rate of tuberculosis patients was 220.7/100 000 in Ruoergai county from July 2003 to July 2004. The levels of TB casefinding in the Duoma district which located in the center and Tiebu and Baxi districts which were semipasturing areas were higher than that in Tangke and Namo districts, which were pasturing areas. The traffic convenience was best in Duoma district when using the tuberculosis health services, while the traffic convenience was worst in Tiebu district. The proportion of TB doctors in first visit doctor was higher in Tiebu and Duoma districts, and the degree of first visit patient holding tuberculosis knowledge was higher in Tiebu district. Conclusions It was crucial for government to attach importance to the control of tuberculosis, health education and improve the geographical accessibility.
