高压氧在带蒂含真皮下血管网超薄皮瓣移植中的应用

摘　　要：目的观察高压氧(HBO)加带蒂含真皮下血管网超薄皮瓣移植用于因工伤及交通事故造成的组织缺损、骨外露修复的治疗效果。方法对120例住院患者(均有组织缺损或骨外露,需行皮瓣移植修复)随机分成3组,每组40例。其中一组用HBO加带蒂含真皮下血管网超薄皮瓣移植治疗,对照组用传统带蒂皮瓣移植及单纯带蒂含真皮下血管网超大型薄皮瓣移植治疗。结果高压氧(HBO)加带蒂含真皮下血管网超薄皮瓣组皮瓣成活率较对照组明显提高(χ2=8.54P<0.01及χ2=3.91,P<0.05),有统计学意义。结论HBO加带蒂含真皮下血管网超薄皮瓣用于组织缺损、骨外露创面修复时皮瓣成活率高、断蒂时间短,应多加推广。Objective Toobserve the therapeutic effect of hyperbaric oxygenation （HBO） combined ultrathin pediculated skin with subdermal vascular net grafting （UPSSVNG） in tissue defect and bone exposure caused by industrial and traffic accident. Methods 120 inpatients with tissue defect or bone exposure that needed prosthesis with pedicle flap were randomly divided into 3 groups. 40 cases in treated group were treated with HBO combined UPSSVNG. 40 cases in control group were treated with traditional pediculated skin grafting, other 40 cases in control group were treatecl with simple UPSSVNG. Results The survival rate of pedicle flap in treated group is obviously higher than in control group, with χ^2 = 8.54, P〈0.01andχ^2=3.91, P〈0.05. Conclusion The treatment of tissue defect and bone exposure with HBO combined IJPSSVNG should be popularized with its high survival rate of pedicle flap and short period before pedicle resection.
