彩色多普勒超声对乳腺增生症的诊断价值

摘　　要：目的评价彩色多普勒血流成像(CDFI)在乳腺增生症诊断中的实用价值.方法分析89例乳腺增生症患者高频声像图特征及频谱特点,观察异常回声形态、部位、大小、边界、有无包膜及钙化、内部回声、彩色多普勒血流信号及频谱多普勒等,并与病理结果进行对比分析.结果 89例中超声检出156个病灶,其中单侧30例,双侧59例.39个病变表现为片状低回声区,33个为类圆形低回声区,9个为不规则低回声区,27个为不规则无回声区伴少许钙化,31个为多发梭形无回声区,17个为类圆形无回声区.内可见丰富彩色血流信号5个,点状彩色血流信号10个,短线状彩色血流信号69个,无彩色血流信号72个.病变＜1 cm 76个,1～2 cm 69个,＞2 cm11个.病理结果:乳腺小叶增生13例,乳腺增生囊肿为主28例,乳腺增生腺病为主14例,小叶增生部分导管上皮增生活跃1例,乳腺增生伴纤维腺瘤形成18例,乳腺导管增殖1例,乳腺小叶增生伴个别导管呈乳头状瘤样增生6例,纤维硬化病8例.结论 CDFI在乳腺增生症诊断中具有重要价值.Objective To evaluate the diagnostic value of color Doppler flow imaging （CDFI） in mammary gland hyperplasia. Methods Eighty-nine patients with mammary gland hyperplasia （ 156 nodules） were evaluated by color Doppler ultrasonography. The abnormal morphology of echoes, location, size, edge, well-filmed or ill-defined border, micro-calcifications, internal echoes, color Doppler blood flow signals and frequency spectrum of Doppler were evaluated. The sonographic and histopathologic findings of all nodules were comparatively analyzed. Results A total of 156 nodules were detected in 89 patients by sonog- raphy. Among them, 30 patients had unilateral lesion, and 59 patients had bilateral lesions. Of the 156 nod- ules, preoperative sonography revealed 39 slice hypoechoic lesions, 33 circular hypoechoic lesions, 9 irregu- lar hypoechoic lesions, 27 irregular non-echoic lesions with a little calcification, 31 multiple fusiform non-e- choic lesions, 17 circular non-echoic lesions. Five nodules were rich in color flow imaging; 10 nodules, in spot -shaped color flow imaging; 69 nodules, in dash-line imaging; and 72 nodules, without imaging. For the lesion size on sonography, 76 nodules measured 〈 1 cm in the longest dimension; 69 nodules, 1 - 2 cm ; 11 nodules, 〉 2 cm. Histopathologic examination revealed that 13 cases of the 89 benign lesions were of lobular multiplication; 28 cases, multiplication mainly with cysts ; 14 cases, multiplication mainly with adenopathy ; 1 case, ductal active multiplication; 18 cases, multiplication with fibroadenoma formation; 1 case, breast ductal multiplication; 6 cases, lobular multiplication with a few papillomatous multiplication; 8 cases, fibro- sclerosis. Conclusions CDFI plays an important role in the preoperative evaluation for patients with mammary gland hyperplasia.
