改良胆肠端侧黏膜吻合法在小口径胆肠吻合术中的应用

摘　　要：目的:探讨小口径胆肠吻合术的新方法,以降低胆肠吻合口瘘与狭窄的发生率。方法:在小口径胆肠吻合术中,应用改良的胆肠端侧黏膜吻合法,Roux-y“桥袢”经胃窦后途经,我们临床实际操作26例。结果:改良吻合法有效地避免了小口径胆肠吻合术术后吻合口瘘与狭窄的发生,解决了小口径胆肠吻合临床操作困难的问题。经胃窦后途经避免了Roux-y“桥袢”相关并发症。结论:改良的胆肠端侧黏膜吻合法和经胃窦后的Roux-y途经在临床上切实可行,安全可靠。Objective： To investigate the new method of small-diameter＇s choledochojejunostomy in order to reduce the occurrence rate of anastomotic fistula and stenosis by it, Methods ： In this process of small-diameter＇s choledochojejunostomy about biliary-to-intestinal tract,we applied the new method of improved end-to-side and the new way of Roux-y＇s “atwalk” to place antrue pyloricum behind. We operated 26 cases actually. Results： This improved choledochojejunostomy＇s method availably avoided the occurrence of anastomotic fistula and stenosis in the operation of small-diameter’s anastomosis, and resolved the difficult problem in operation of small-diameter’s choledochojejunostomy. The placed behind antrue pyloricum avoided related complications about Roux-y＇s “atwalk”. Conclusion： Both of improved small-diameter＇s choledochojejunostomy with homologous mucous membrane and end-to-side method with putting behind antrue pyloricum were practical and safe.
