心脏舒张功能不全中医辨证分型聚类研究

摘　　要：目的研究心脏舒张功能不全患者的中医证候特点及其与相关症状、舌、脉的关系,从而确立中医辨证分型的分布规律。方法采用SPSS10.0版统计软件包,对86例心脏舒张功能不全患者的症状、舌、脉进行K-means聚类分析后,再对每种类型进行主成分分析,判定每种类型的代表症状,归纳出证型。结果调查收集的心脏舒张功能不全患者的症状共有26种,症状频率出现较高的是头昏、头晕、心悸、气短、胸闷、乏力倦怠、口干、口苦、纳呆、痰多;症状频率出现较低的是脘闷、腰痛、腰酸腿软、头痛、口唇紫绀、尿频、形体肥胖、畏寒肢冷。舌质以以淡红、暗红为主;舌苔以白、腻为主。常见的脉象是细脉、弦脉、滑脉。总结出4大证候群。结论根据聚类分析结果并结合临床实践,认为心脏舒张功能不全患者应分为四大证候群。Ⅰ型:心脾两虚、气血亏虚型;Ⅱ型:心肾阳虚、痰瘀水互结型;Ⅲ型:肝肾阴虚、肝阳上亢型;Ⅳ型:脾胃气虚、痰浊中阻型。Objective To explore the characteristics of TCM manifestation in patients with heart diastolic insufficiency and relationship between the syndrome characteristics and symptoms, the tongue appearances and pulse conditions. And to sum up the distribution rules of syndrome type of TCM in patients with heart diastolic insufficiency. Methods The results were analyzed by SPSS 10.0 software. K-means cluster analysis was used to classify the symptoms, tongue appearance and the pulse condition of 86 cases of heart diastolic insufficiency. Every syndrome type of TCM was analyzed by principal component analysis and the representative symptoms were determined. The syndrome type of TCM in patients with heart diastolic insufficiency was concluded. Results There were 26 symptoms in patients with heart diastolic insufficiency. Dizziness, palpitations, shortness of breath, oppression in chest, lack of strength and listlessness, thirst, bitter taste in mouth, anorexia, expectoration of much sputum were present frequently. Gastric stuffness, lumbar pain, soreness and weakness in lumbar and knee, cyanotic lip, obesity, headache, frequent urination, intolerance of cold and cold feeling in limbs, were present rarely. The principal tongue body was pale-red tongue and dark-red tongue, and the principal tongue coating was white coating and greasy coating. The common pulse conditions were thready pulse, wiry pulse, slippery pulse. Four syndrome types of TCM are concluded. Conclusion Integrating the results of cluster analysis with clinic practice, we concluded that the syndrome types of TCM in patients with heart diastolic insufficiency could be summed up into four TCM syndrome types. I type is insufficiency of both heart and spleen, syndrome of both qi and blood deficiency. Ⅱ type is deficiency of heartkidney yang, syndrome of combined stagnant-phiegm and edema.
