小剂量卡维地洛治疗老年慢性心力衰竭的疗效观察

摘　　要：目的探讨小剂量卡维地洛对老年(≥60岁)慢性心力衰竭(CHF)患者疗效。方法将60例经常规心衰治疗病情稳定的老年CHF患者随机分为两组。卡维地洛组30例,在原有治疗基础上加服卡维地洛3.125mg,每日两次;持续2周后,无明显不良反应者加量至6.25mg,每日两次,直到6个月。常规治疗组30例作为对照。分别检测两组患者治疗前后心率(HR)、左室射血分数(LVEF)和左室后壁(LVP)、室间隔(IVS)厚度。结果与常规治疗组相比,卡维地洛组患者的HR变慢,LVEF增加,LVP和IVS厚度均减少。结论卡维地洛治疗老年CHF6个月后,具有逆转左室重构和改善心功能的有益作用。Objectives：To study the clinical therapeutic effects of small dosage of carvedilol on chronic heart failure（CHF） in old patients（≥60 y）. Methods：Old patients （n=60） who tolerated conventional treatment of CHF（diuretics, ACEI, digitalis） were randomly divided into 2 groups. The carvedilol group,30 patients （M19, F11） were treated with carvedilol 3. 125mg bid for 2 weeks, those without obvious adverse reactions received 6.25mg bid for 6 months. The conventional group, 30 patients （M15, F15） still received conventional teatment as control, We detected patients＇ heart rate （HR） ,left ventricular ejection fraction（LVEF）, thickness of left ventricular posterior wall（LVP） and interventricular septum（IVS） before and after treatment. Resuits：Compared with the conventional treatment group the HR, thickness of LVP and IVS decreased, LVEF increased in the carvedilol group after 6 months treatment. Conclusions： It has good clinical effects on reversing the left ventricular remodeling and promoting the cardiac function after 6 months treatment of carvedilol in old CHF patients.
