摘  要:【目的】探讨早产儿与足月儿早期系统促进的模式及其效果,为早期发展促进工作提供依据。【方法】足月儿以“四二一“查体程序为框架,早产儿明显加大指导和促进密度,在常规保健基础上进行运动、认知、环境等方面早期促进活动。【结果】1岁时,早产儿促进组体重、身长已经赶上甚至超过足月儿对照组,而早产儿对照组体重和身长与足月儿两组之间差仍有显著性。早产儿促进组发育商(developmentalquotient,DQ)在1岁和2岁时分别高出对照组5.62和8.53分;早产儿促进组DQ在3个月和1岁时已经赶上足月儿对照组。足月儿促进组DQ在3个月、1岁、2岁时分别高出对照组3.57、3.09和3.77分。【结论】以儿童保健机构为依托、以家庭为核心,评估-促进-再评估-再促进的模式是经济有效的促进模式,对早产儿体格发育和智力有一定促进作用。[Objective] To prohe into the model and it's effects of early systemic promotion on premature and mature infants. [Method] The program was carried out with motor, cognitive and environmental promoting measures by teaching the parents , based on the procedure of “4-2-1” physical examination on mature infants and a much more frequent promoting on premature infants. [Results] The premature-promotion infants had caught up with the mature infants in weight and height by 1 year old, while the premature-control infants still had a lower weight and height than that in mature infants. Premature-promotion infant's developmental quotients(DQ)were 5.62 and 8.53 higher than those in premature-control infants by the 1 year and 2 years old, respectively , and have already caught up with mature-control infants by 3 months and 1 year old. The DQ of mature-promotion infants were 3.57,3.09 and 3.77 higher than those of mature-control infants by 3 months, 1 year and 2 years old, respectively. [Conclusions] These early promotion activities and their frequency are suitable to both premature and mature infants. It is an cffective and economic early promotion model based on the procedure of evaluating-promotion, and re-evaluating and re-promotion ,home-center supported by child health-care organization.
