鼓室表皮样结构角蛋白表达的变异

摘　　要：为探讨鼓室表皮样结构（ＥＦ）与先天性胆脂瘤发病的关系，收集１６例鼓室ＥＦ标本，其中单纯型１５例，增殖型１例，采用ＣＫ１３、ＣＫ１８、ＣＫ１９和ＰＫ四种角蛋白抗体，通过免疫组织化学染色方法，对比观察了鼓室ＥＦ、外耳道表皮和鼓室粘膜上皮的角蛋白表达。结果显示，单纯型ＥＦ的角蛋白染色特点与鼓室粘膜上皮一致，而不同于外耳道表皮，提示单纯型ＥＦ不具有典型的表皮特征；增殖型ＥＦ的角蛋白表达出现变异，ＣＫ１９的表达完全缺失，与鼓室粘膜上皮形成鲜明对照；ＰＫ染色特点与外耳道表皮相似。提示增殖型ＥＦ具有某些表皮特点。此结果进一步表明增殖型ＥＦ可能是先天性胆脂瘤的前体。Theepidermoidformation(EF)inthedevelopinghumanfetalmiddleearisbelievedtohaverelationshipwithcongenitalcholesteatoma.SixteenEFswereincludedinthisstudy,ofwhich15weresimpleEFsandonewashyperplasticEF.FourcytokeratinantibodiesPK,CK13,CK18,andCK19wereusedinanABCimmunohistochemicaltechniquetocomparetheexpressionofthecy-tokeratinintheEFs,middleearmucosaandexternalacousticmeatusskin.TheresultsshowedthatthesimpleEFshadthesamestainingpatternofmiddleearmucosaforalfourantibodiesandwerediferentfromexternalacousticmeatusskin.ItwassuggestedthatthesimpleEFsdonothavethepropertiesofanepidermis.ThehyperplasticEFhadanalteredcytokeratinexpressioncomparingtothatofthemiddleearmucosa.TheexpressionofCK19disappearedinthestructurewhilethenormalmiddleearmucosanearbyhadapositivestainingforthisantibody.PKstainingpattern,characterizedbythemoreintensiveinthesurfacelayeroftheEF,wassimilartothatofexternalacousticmeatusskin.ItissuggestedthathyperplasticEFmayacquirecertainpropertiesofepidermis,andmaydevel-opintocongenitalcholesteatoma.
