沙美特罗替卡松吸入剂预防急性高原反应的效果观察

摘　　要：目的:观察沙美特罗替卡松粉吸入剂预防急性高原反应的效果。方法:55名青年随机分为沙美特罗组(n=31)和安慰剂组(n=24),自海拔1400m历时4天进入5200m,第2天开始,沙美特罗组每天早晚各吸入沙美特罗替卡松粉一个剂量(沙美特罗50μg,丙酸氟替卡松100μg),连续7天;安慰剂组用同样方法吸入少量生理盐水。以军用卫生标准GJB1098-91《急性高原反应的诊断和处理原则》随访观察第2天、4天、7天急性高原反应的严重程度、SaO2和脉率。结果:进入海拔5200m第7天安慰剂组急性高原反应症状较沙美特罗组严重(P<0.05),第2天、4天安慰剂组脉率较沙美特罗组减慢(P<0.05),SaO2两组无显著性意义(P>0.05)。随访期间沙美特罗组发生一例高原脑水肿,安慰剂组发生一例高原肺水肿。结论:沙美特罗替卡松粉吸入剂预防急性高原反应无确切效果。Objective： To assess the effects of salmeterol xinafoate and fluticasone propionate powder for inhalation on the incidence of acute mountain sickness. Methods： In this double - blind, randomized, placebo - controlled trial, 55 subjects residing at 1 400m inhaled one dose salmeterol（ 50 gmc salmeterol and 100 mcg fluticasone propionate） or placebo（0. 9% sodium chloride） twice daily after ascending at an altitude of 5 200m. The effects of altitude were measured with the Acute Mountain Sickness Self - report Questionnaire Scoring System and resting SaO2 and heart rate at the 2nd, 4th and 7th day at the summit. Results： The sahneterol （ n = 31 ） and placebo （ n = 24） groups were well matched in weight, height, or age. The subjects treated with salmeterol had a statistically significant reduction in the score of acute mountain sickness at the 7th day （ P 〈 0.05）, compared with subjects who received placebo. Measured oxygen saturation was similar between subjects randomized to receive ginkgo compared with placebo at the 2nd and 4th day （P 〉 0.05）, however, the heart rate wassignificantly lower in placebo than in sahneterol（P 〈0.05） . One subject on sahneterol group and one on placebo developed high altitude pulmonary edema and required descent for their safety. Conclusions： It should be explored further in more detail whether salmeterol is an effective prophylactic agent for acute mountain sickness.
