摘 要:为研究小儿围体外循环期血浆离子钙浓度的变化规律,指导临床合理地应用钙剂,连续测定了60例先天性心脏病病儿血浆离子钙浓度[Ca2+]、pH、红细胞压积(HCT)。结果表明,在pH相对恒定的情况下,[Ca2+]在转流15分钟时最低,此后逐步升高,转流后2小时同转流前、24小时与48小时高于转流前水平。转流中的[Ca2+]经HCT校正后明显高于转流前。婴幼儿与年长儿、紫绀组与非紫绀组之间无差异。结论:小儿围体外循环期有内源性[Ca2+]产生,一般无需补充钙剂。Aim:This study was designed to observe the aiteration of plasma ionised calcium concentration [Ca2+] in children undergoing cardiopulmonary bypass(CPB).Materials and Methods:The[Ca2+],pH and hematocrit were assayed in 60 patients with congenital heart diseases.Results:The results showed that the level of[Ca2+]was lowest at l5 minutes after the start of CPB,then elevated gradually,and finally returned to the pre-bypass leve l2 hours after the termination,and to a higher lewel 24 hours and 48 hours post-CPB. But,the corrected value of [Ca2+] by hematocrit during CPB was significantly higher than that of pre-CPB.Conclusion:Because of endogenous release of [Ca2+]our daba indicate that it is nor necessary to supple calcium salt during and after CPB because of the preseuce of endogenous release of[Ca2+]。
