颈椎板成形椎管扩大术及椎板间植骨融合治疗颈椎管狭窄症

摘　　要：为了使扩大的椎管达到牢固的骨性愈合，在颈椎板成形椎管扩大术中对８７例病人行活页侧椎板间植骨融合。随访１～４ａ，按ＪＯＡ标准评定疗效。结果：优２７例，良４１例，改进１６例，无变化３例，有效率为９６．５％。提示：行椎板间植骨融合，可使扩大的椎管达到牢固的骨性愈合，防止椎板塌陷，由于植骨材料取自剪下的棘突，所以不增加手术的难度及病人的痛苦。The purpose of this study was to obtain the stable bony union in the enlarged vertebral canal.87 patients were treated with bone grafting between the vertebral laminae in the operation of cervical laminoplasty.The follow-up of 1 to 4 years showed satisfactory results according to the criterion of JOA.There was 27 cases excellent with postoperative improvement rate>80%,41good(50%～79%),16 improved (5%～49%),3 poor(<5%).The average rate of improvement was 96.5%.It is concluded that we made use of the bone grafting between the vertebral laminae with the cut spinous process to obtain the stable bony union in the enlarged vertebral canal,thus preventing collapse of the vertebral laminae.Thanks to the cut spinous process for the bone grafting material,there is no increase in the difficulty of operation and pain for the patients.
