局部注射曲安奈德治疗耳垂瘢痕瘤

摘　　要：目的:通过对19例(38耳)耳垂瘢痕进行局部注射长效激素曲安奈德治疗,总结耳垂瘢痕瘤治疗的经验.方法:用注射针头抽吸曲安奈德沿着耳垂瘢痕瘤的边缘组织进针,每一点注射药物0.1～0.2ml,每15～20d注射1次,共注射6～7次,时间为3～4个月,若瘢痕已软化松解,即可停止注射治疗,若不理想再追加注射1～2次.结果:19例患者注射3～4次后瘢痕瘤开始变软,并逐渐松解,当注射6～7次后瘢痕组织基本松解,停1～2个月后复查,若瘢痕已彻底软化松解,即可停止治疗,若不理想再追加注射药物1～2次.大于2cm以上的瘢痕瘤不易完全松解.结论:手术方法松解瘢痕失败的原因,就是这些方法都要在耳垂部留有新的创面,造成了新的感染机会,给新创口带来了各种抗原,而大于2cm以上的瘢痕瘤不能完全松解,必须先通过手术切除大部分瘤体再行注射.本法通过长效激素曲安奈德的局部注射,避免了新的变态反应的发生,而且通过曲安奈德在局部高效的抗炎、抗变态反应的作用逐渐松解了原已形成的瘢痕.Objective：To summarize the experience of triamcinolone acetonide by local injection in treating keloma of ear lobe, Methods：19 cases （38 eyes） with keloma of ear lobe were treated with locally injecting triam- cinolone acetonide 0.1-0,2ml every 15-20 days for 3-4 months until the keloma was soluted. One or more injection were added for unsatisfied effect. Results： Keloma were softening after 3-4 times of injection and most of them were soluting after 6-7 times of injection. If the rechecking outcome was unsatisfied with discontinuation 1 - 2 months, triamcinolone acetonide 1 - 2 times were needed. The efficacy of keloma in diameter over 2 cm was lower. Conclusion： Triamcinolone acetonide contributes to solute the keloma of ear lobe with antiflammatory and anti - allergic effect.
