摘 要:目的观察水蛭素注射液对自发性脑出血血肿吸收、血肿周围低密度区改变及对神经功能恢复的影响。方法将465例入选的自发性脑出血患者随机分为治疗组(308例)和对照组(157例)。治疗组给予水蛭素注射液,6ml加入5%葡萄糖液250ml中静脉滴注,1次/d。分别于发病第1、4、10、21天利用CT测量血肿体积及血肿周围低密度区,采用欧洲卒中量表进行神经功能缺损评分。结果治疗组血肿体积第10天为(21±10)ml,第21天为(14±7)ml,对照组第10天为(27±8)ml,第21天为(25±9)ml,两组比较差异有显著意义;治疗组在20d时血肿周围密度区为1·5mm,对照组为3mm,两组比较差异有显著意义;治疗组第10、20天欧洲卒中量表评分分别为62±19、66±24,对照组分别为46±16、51±14,两组比较差异有显著意义,P<0·05。结论水蛭素注射液能促进脑出血血肿的吸收,使血肿周围低密度区缩小和促进神经功能恢复。Objective To investigate the effect of hirudin on absorption of intracerebral hematoma, changes of low density area around hematoma, and recovery of neurological function in patients with spontaneous intracerebral hemorrhage. Methods 465 patients with spontaneous intracerebral hemorrhage were randomly divied into treatment group and control group, 308 cases and 157 cases respectively. The patients in treatment group were intravenously given the mixture of hirudin solution 6 ml and 5% glucose 250 ml once daily. The volume of hematoma and low density area around hematoma were measured by CT at 1, 4, 10 and 21 days after teatmeat, and the neurological function was scored by the European Stroke Scale. Results Hematoma volumes in the treatment group were (21 ± 10) ml and ( 14 ± 7) ml at the 10th and 20th day, and in the control group were (27 ± 8) ml and (25 ± 9) ml respectively. Hematoma volume in the treatment group was smaller than that in the control group, The difference was statistically significant, The low density area around hematoma in the treatment group was 1.5 mm and that in the control group was 3 mm in thickness. The neurological fnnction score in the treatment group was higher than that in the control group at 10th and 20th day. Conclusion Hirudin can enhance absorption of hematoma and recovery of nervous function and reduce tow density area around hematoma in patients with spontaneous intracerebral hemorrhage.
