糖尿病患者血浆内皮素水平升高和临床意义

摘　　要：采用放射免疫分析法测定了３３例正常人及５７例糖尿病患者血浆内皮素水平，结果显示，胰岛素依赖型糖尿病（ＩＤＤＭ）及非胰岛素依赖型糖尿病（ＮＩＤＤＭ）患者内皮素水平均显著高于正常人，伴微量白蛋白尿的患者内皮素水平进一步显著升高，患者内皮素水平与其尿白蛋白排泄率及视网膜病变呈显著正相关。表明糖尿病患者血浆内皮素水平升高与糖尿病时微血管病变有密切关系，提示内皮素可能在糖尿病血管病变机制中有重要意义。Plasma endothelin-1 levels in 57 diabetic patients and 33 normal subjects were measured by radioimmunoassay. The results showed that the plasma endothelin-1 levels in both IDDM and NIDDM were significantly higher than that in normal subjects(P<0.001).The plasma level in patients with microalbuminuria was also significantly higher than that of those with normal urinary albumin excretion rate(P<0.05).In patients with retinopathy but a normal uAER, the endothelin-1 level also tended to increase, but was not statistically significant. In the studied group, the plasma endothelin-1 level was positively correlated with uAER and retinopathy. The results indicate that the elevated plasma endothelin-1 level in patients with diabetes mellitus was closely related to microangiopathy and suggest that endothelin-1 might play a role in the pathogenesis of diabetic angiopathy in diabetic patients.
