摘  要:目的:探讨腰椎横突骨折及其并发伤的CT表现特征和意义。方法:结合X线平片,对83例腰椎横突骨折及其并发伤的CT表现特征进行回顾性分析。结果:83例共计163个横突骨折中,常规X线平片漏诊112个(68.7%)。CT表现可分3种类型:横形骨折(Ⅰ型)33例65个,斜形骨折(Ⅱ型)31例60个,撕脱性骨折(Ⅲ型)19例38个;并发胸腹部脏器伤61例(73.5%);L1、L2高位横突骨折与胸部胸膜肺并发伤具有明显相关性(P<0.0001),多发性腰椎横突骨折组的脏器并发伤发生率明显高于单发性骨折组(P<0.0001)。结论:在腰椎横突骨折的显示上,CT优于常规X线平片,其胸腹脏器并发伤的发生率与腰椎横突骨折类型无关,而与骨折所在腰段的部位和多发性有关。腰椎横突骨折可作为胸腹脏器并发伤的重要提示性征象。Objective:To investigate the type of fracture of lumbar transverse process on CT and its concurrent injury. Methods:Combining with the plain radiographs,the CT features of fracture and its concurrent injuries of 83 cases of lumbar transverse process were analysed retrospectively. Results:From 163 fractures of 83 cases, 112 were missed by routine Xray examination (68.7%). The injuries were divided into 3 types:65 (33 cases) for transverse fracture ( type Ⅰ ) ,60 (31 cases) for oblique type fracture (type Ⅱ),38 (19 cases) for tearing off fracture (type Ⅲ).61 cases (73.5%) had concurrent abdominal and chest organ injuries. The fracture of 1st and 2nd lumbar transverse process and the adjacent chest concurrent injuries had significantly relativity (P〈0. 0001). The incidence of concurrent injuries in multiple lumbar transverse process fracture was obviously higher than in solitary one (P〈0. 0001). Conclusion:The CT is better than the plain radiograph in displaying lumbar transverse process fracture. The incidence of the thoracic and abdominal organ concurrent injuries for the lumbar transverse process fracture has no relativity to the type of fracture but to the site and numbers of fracture. So, the lumbar transverse process fracture is a important sign of the thoracic and abdominal organ concurrent injuries.
