经颈内静脉肝内门体分流术操作中的经验和教训

摘　　要：１９９３年７月～１９９５年１月，作者对１１０例晚期肝硬化门静脉高压症患者实施ＴＩＰＳＳ治疗，成功率为９５．１５％（７例门静脉血栓放弃ＴＩＰＰ操作）。与ＴＩＰＳＳ操作有关的并发症发生率为２．９１％，并发症包括腹腔内出血２例，右侧胸腔内出血１例；术后１个月内死亡率是３．８８％，死亡原因是急性肝功能衰竭和食管静脉曲张再出血，无１例与ＴＩＰＳＳ操作有关。１～１９个月随防，出血、腹水复发５例，肝功能衰竭死亡３例。肝癌死亡２例。作者总结ＴＩＰＳＳ操作的经验，讨论与ＴＩＰＳＳ操作有关的几个问题。Abstract From July, 1993 to January, 1995. we performed transjugular intrahepatic portosystemic stent shunt (TIPSS) in 103 consecutive patients of advanced cirrhosis and portal hypertersion, with the technical success rate of 95. 17%. The complication included intraperitoneal hemorrhage in 2 cases and right thoracic hemorrhage in one. The operative mortality after within 30 days was 3. 88%. Three patients died of hepatic failure and one died of variceal rebleeding. At follow-up of 1￣19 months,variceal rebleeding and ascite were found in 5 patients. Late death were due to hepatic failure in 3 patients and to hepatic carcinoma in 2 patients.
