人类白细胞抗原与IgA肾病关联及其临床意义

摘　　要：７５例经肾穿刺证实ＩｇＡ肾病患者来自上海地区汉族患者检测了ＨＬＡ－Ａ、Ｂ、ＤＲ抗原和５９例ＨＬＡ－ＤＱ抗原，发现ＨＬＡ－ＤＲ４抗原频率明显增高（Ｐ＝０．０００００２），ＨＬＡ－ＤＲ５表型频率降低（Ｐ＝０．０００６２）；ＨＬＡ－ＤＲ４抗原阳性者在大量蛋白尿组和肉眼血尿组均增高。Seventy-five adult chinese patients with biopsy-proved primary IgA nephropathy were typed for HLA-A, B and DR antigens,and 59 of these cases were also typed for HLA-DQ antigens. All patients were from shanghai region and of han nationality,A significant association with HLA-DR4 was clearly noted (P=0.000002); A lower phenotype freqency (PF) P=0.00062 of HLA-DR5 was observed in the patients.The PF of HLA-DR4 was similarly high in the massive proteinuria group and in the hematuria macroscopic group.Its prognostic value remains to be confirmed,due to the relatively small size of the sample and insufficient follow-up time involved in our research.
