摘　　要：一般认为,年门诊量超过60万人次的《门诊挂号系统》如果设备投入、系统设计不到位,就容易产生“瓶颈效应”,这是国内一些医院信息化建设方面的资深人士所达成的共识和经验之谈。自1999年我院全面启动建设《医院信息管理系统》以来,门诊挂号系统的建设就是整个HIS系统中最难攻克的一个堡垒。不是因为技术上的难度,是因为容易形成“瓶颈效应”的特殊性。为攻克堡垒,信息中心从两个方面入手,一是对软件的优化,二是提高挂号员的计算机操作水平。对挂号软件进行了优化并新增了启用门诊就诊卡(磁卡),将病人的基本信息在挂号、收费、药房各个环节中共享;新设了病人预诊卡,病人在挂号之前可进行前台的分诊;提供了高效的项目检索码;设立了少数民族常见姓名字典库;打印专家所在楼层号:知名专家的限号、今日停号等功能,大大节省了录入的时间。在《门诊挂号系统》的正式启动日,准备了应急方案,并在就诊高峰期启动应急方案。该项目的实施,终于结束了我院40多年手工挂号的历史。是对我院《医院信息管理系统》的进一步完善,它的启用为我院门诊医生工作站的实现奠定了坚实的基础。Generally, for a clinic register system, whose annual clinic amount exceeds 600,000 people, the effect of the bottleneck would appear if equipment investment and design are not complete systematical, as is the common understanding of the senior experts in hospital information con- struction domestically. Since 1999, our hospital has built The Hospital Information Management System. The clinic register systematic is a difficult point in the whole HIS. It is not because of its difficult in technique, but it is easy to form bottleneck effect. In order to solve the problem, the information center did a hard work. First, we improved the software; then we improved the register＇s computer operation level. The register software consists of basic information of patient （the magnetic stripe card）. The patients＇ basic information can be shared between registration, charge and pharmacy. With the patients＇ examine card, the patients can be divided at the front desk based on their goals of before registering. The system offers the high-efficient search codes for the items; it sets up the common name dictionary storehouse for the minorities; it is of the functions of typing the expert＇s floor, limitation of his consultation and out of service. The time for inputting is greatly saved. We designed a contingency solution which is used for an urgent need in the peak of the amount of outpatients. The implementation of the system finished registration by hands in more than 40 years in our hospital. It is very important and sets a solid foundation to make information management system of the hospital much better, and realized the clinic doctor＇s work station of our hospital.
