摘 要:目的探讨实时三维超声心动图(3DE)估测左室质量的可行性和准确性。方法应用Philips实时3DE采集19只离体猪心金字塔形数据库,结合相应测量软件用心尖长轴系列平面法分别测量左室壁质量,并与二维超声(2DE)双平面Simpson法和离体猪心排水法实测值比较。结果实时3DE16平面法(201.92±26.43)g、8平面法(201.84±27.60)g估测左室壁质量值与猪心排水法实测值(206.79±26.74)g无统计学差异(P>0.05),而2平面法(167.74±22.15)g和2DE双平面Simpson法(183.54±24.54)g测值与排水法实测值有统计学差异(P<0.05);实时3DE16平面(r=0.92,P=0.0001)和8平面法测值(r=0.91,P=0.0001)与排水法实测值高度相关,2DE双平面Simpson法则较逊(r=0.74,P=0.0001)。结论实时3DE是估测左室壁质量的可靠手段。Objective To evaluate the feasibility and accuracy of real-time three-dimensional echocardiography(3DE)for quantitative analysis of left ventricular mass. Methods Nineteen excised porcinehearts were examined by real-time 3DE and conventional two-dimensional ecnocardiograpny (2DE) With Philips Sonos 7500 echocardiographic system. Left ventricular wall mass examined by real-time 3DE was outlined and calculated with 2-plane, 4-plane, 8-plane and 16-plane method using the 4D Cardio-View RT of Tom Tec image system, and the results were compared with those examined by 2D biplane Simpson-method. The true value of left ventricular wall mass measured by water displacement served as a reference standard. Results “Full volume” imaging in all excised porcine hearts was well displayed by real-time 3DE. There was no significant difference (P 〉 0.05) between the values of left ventricular wall mass examined by real-time 3DE 16-plane (201.92)± 26.43)g,8-plane(201.84±27.60) g and that by water displacement (206.79 ±26.74) g, but there was significant difference(P〈0.05) between the values by real-time 3DE 2-plane(167.74±22.15) g, 2D biplane Simpson-method (183.54± 24.54) g and that by water displacement. Meanwhile, the values of left ventricular wall mass measured by RT-3DE 16-plane(r = 0.92, P =0.0001)and 8-plane( r = 0.91,P = 0. 0001)were highly correlated with that by water displacement. The correlation between 2DE biplane Simpson-method and water displacement( r = 0.74) was less significant for left ventricular wall mass measurement than that between 3DE 16-plane, 8-plane and water displacement. Conclusions Real-time 3DE may be a valuable and accurate technique for quantitative analysis of left ventricular wall mass.
