宫颈癌腹腔镜手术后患者的心理状况及影响因素

摘　　要：目的探讨腹腔镜宫颈癌手术后患者的心理状况及影响因素。方法运用症状自评量表(SCL90)评定宫颈癌腹腔镜术后患者的心理状况并通过自行设计的多项目调查表综合分析其影响因素。结果宫颈癌腹腔镜术后患者SCL90躯体化、焦虑、敌对、恐怖、偏执、精神病性6个因子明显高于国内女性常模(P<0.05);小于35岁以下患者的人际关系、抑郁因子明显高于35岁及以上患者(P<0.05)。高学历组患者的躯体化、焦虑明显高于低学历组(P<0.05)。家庭关系欠佳患者强迫、抑郁、偏执因子明显高于家庭关系较好患者(P<0.05)。低收入组抑郁因子明显高于高收入组(P<0.05)。腹腔镜术后有并发症患者的躯体化、人际关系明显高于术后无并发症组(P<0.05)。结论宫颈癌腹腔镜术后患者有较明显的心理问题。其心理状况低下与年龄、文化程度、家庭收入、家庭关系、手术并发症有关。Objective To study psychological state and its related factors of cervical carcinoma patients after laparoscopic operation. Methods Symptom check list （SCL-90） was employed and the self-developing questionnaires were used to investigate the related factors. Results In SCL-90, the scores of the somatization, anxiety, hostility, phobic anxiety, paranoid ideation and psychotism were significantly different with that of the Chinese female normal （P 〈0.05）. In the patients less than 35 years, the scores of interpersonal sensitivity, depression were significantly higher than those more than 35 years （P 〈 0. 05 ） ; In the patients of high education, the scores of somatization, anxiety were significantly higher than those of low education （ P 〈 0. 05 ） ; In the patients of bad family relationship, the scores of obsessive-compulsive, depression, paranoid were significantly higher than those of good family relationship （ P 〈 0. 05 ） ; In the patients of low income, the scores of depression were significantly higher than those of high income （ P 〈 0. 05 ） ; In the patients with postoperative complications, the scores of somatization, interpersonal sensitivity were significantly higher than those without postoperative complications （P 〈0. 05）. Conclusion The psychological state of cervical carcinoma patients after laparoscopic operation is poor. The poor psychological state of cervical carcinoma patients is related with age, education level, income, family relationship and postoperative complications.
