sE-SLT与LPO的测定在老年肺炎中的应用

摘　　要：为探讨可溶性E-选择素(sE-SLT)、过氧化脂质(LPO)在老年肺炎中的作用和临床意义,采用定量酶联免疫吸附实验双抗体夹心法和八国木夫硫代巴比妥酸比色法测定60例老年肺炎患者和30例正常人血浆sE-SLT及血清LPO含量,肺炎患者根据PORT评分系统分为Ⅲ、Ⅳ、Ⅴ3组,结果显示老年肺炎患者sE-SLT、LPOⅢ组为44.73±4.83μg/L、4.49±0.21nmol/mL;Ⅳ组为55.31±5.21μg/L、4.86±0.11nmol/mL;V组为83.83±9.45μg/L、4.88±0.38nmol/mL;明显高于正常对照组(P<0.001),3组间比较sE-SLTⅢ组与Ⅳ组之间有差异(P<0.05),LPOⅢ组与Ⅳ组之间差异显著(P<0.01),V组与Ⅲ组、Ⅳ组肺炎患者比较sE-SLT、LPO有非常显著差异(P<0.001)。本试验组PORT评分>130者sE-SLT在83.83±9.45μg/L范围,提示sE-SLT>74.38μg/L可作为重症肺炎的指标之一。结果提示老年肺炎血浆sE-SLT、血清LPO的变化是反映感染性疾病的良好指标,且与病情严重程度存在明显相关性,其含量变化对于观察老年肺炎病情进展、严重程度、治疗效果及预后具有临床意义。To observe the effects of E-selectin（E-SLT） and lipidoxidase（LPO） in senile pneumonia, 60 cases and 30 healthy people were collected, sE-SLT was determined by ELASE and LPO by colorimetry, all cases were graded into Ⅲ, Ⅳ, and Ⅴ according to PORT evaluation system. The results showed the sE-SLT and LPO in pneumonia cases is higher than healthy people before treatment （P 〈 0. 001）, there was difference in sE-SLT and LPO between pneumonia cases Ⅲ and Ⅳ, and there were obvious differences in sE-SLT and LPO between Ⅴ and Ⅲ, Ⅳ before treatment （P 〈 0. 001）. All of these indicate that E-SLT and LPO are good indexes of reflecting infections, there is obvious relationship between them and degree of disease. Monitoring their contents has clinical meaning for patient＇s condition, degree of disease, therapeutic efficacy, prognosis of senile pneumonia. The contents of sE-SLT in pneumonia cases V fluctuate between 83.83 ± 9.45 洪旭初（1958-）,男,副主任医师,主要从事呼吸系统疾病的临床研究μg/ 1 if PORT〉 130 in this experiment, so it was indicated the content of sE-SLT can be one of diagnosis indexes for severe pneumonia when sE-SLT 〉74.38 μg/1.
