儿科病人自动出院的原因、影响及对策探讨

摘　　要：目的探讨儿科病人自动出院的原因、影响及对策。方法分析本院儿科2年内自动出院的病人116例。结果同期儿科出院7009例,自动出院率1.6%,新生儿科的自动出院率2.63%,显著高于普通儿科的1.33%(x2=13.71,p<0.05);自动出院者多见于新生儿(39.66%)、呼吸系统(19.83%)和血液系统疾病(12.07%);其疾病状况为好转、未愈和病危的分别占49.15%、38.79%和12.07%;自动出院原因的前3位是误解病情(41.38%)、经济困难(26.72%)和怀疑医术(15.52%)。结论自动出院对病人及其家庭、医院和社会均造成负面影响,应针对病情好转和未愈的新生儿、呼吸和血液系统疾病患儿,加强健康教育和医患沟通,减轻患者的负担,不断提高技术水平和服务质量,完善社会救助机制和建立自动出院者的管理制度,以减少自动出院及其负面影响。Objection The aim of this study was to investigate the causes, effects and countermeasures of patients left hospital without cure in pediatric department.Methods 116 cases of pediatric patient left hospital without cure in 2003 to 2004.were analysed.Results There were 7 009 patients in pediatric department left hospital in the same period of time.The rate of patients left hospital without cure was 1.66%, that rate in neonatal department （2.63%） was higher than that in pediatric department （1.33%, X^2=13.71, p〈0.05） .The diseases of patients left hospital without cure were common in neonates （39.66%）, respiration system （19.83%） and blood system （12.07%） .The statuses of patients left hospital without cure were straighten （49.14%）, not heal （38.79%） and sick into death （12.07%） .The causes of patients left hospital without cure were misapprehend （41.38%）, poor （26.72%） and unbelive doctor （15.52%） .Conclusion That patients left hospital without cure could made negative effects to themselves, their family, the hospital and society.In order to reduce the rate of patients left hospital without cure, especially in neonatal patients and child suffer from respiration or blood diseases, we must strengthen the health education and the communication between patients and doc- tors, lighten the burden of patients, heighten the medical technique and service.The society salvation and the adminis- tration of patients left hospital without cure must to be set up.
