摘 要:目的:探讨血粘度、红细胞变形性与老年2型糖尿病胰岛素抵抗性的关系。方法:对42例老年2型糖尿病患者,男23例,女19例,(68.2±7.3)岁,行葡萄糖耐量、胰岛素释放试验及短时胰岛素耐量试验,测算胰岛素敏感性指标KITT值,并进行血流动力学、细胞动力学指标检测。结果:全血粘度穴BV雪低、中、高切粘度均明显高于正常穴P<0.01雪,全血还原粘度穴RBV雪、低切粘度增高穴P<0.01雪。红细胞最大变形指数穴DImax雪降低而红细胞最大聚集指数穴AImax雪增高穴P<0.01雪,行多元偏回归系数分析,BV、RBV低切、DImax与KITT值密切相关穴P<0.05雪。结论:老年2型糖尿病患者血粘度增高和红细胞变形能力降低可以增加代谢物质血流传输障碍,加重胰岛素抵抗。Objective: To investigate the relation between erythrocyte defomability, blood viscosity and insulin resistance in aged type 2 diabetics. Methods: Fourty-two patients with type 2 diabetics(23 males, 19 females, aged 68.2±7.3 years) were measured the oral glucose tolerance test, insulin releasing test, blood lipids and short time insulin tolerance test (calculate Kitt value), and examined the blood rheology items such as blood viscosity in different shear speed, cytorheology parameters DImax, AImax (deformation index, aggregation index). Results: The blood viscosity(BV) of three shear rates(10 s^-1,60 s^-1,160 s^-1) and the reduced blood viscosity(RBV) of two shear rates(10 s^-1, 60 s^-1)elevated. The erythrocyte max deformation index(DImax) lowered, while the max aggregation index (AImax) increased. By multiple linear regression analysis, Kitt value was found correlated with BV(10 s^-1), RBV(10 s^-1) and DImax. Conclusion: High blood viscosity and impaired erythrocyte deformability in erythrocyte membrane may increase resistance to insulin in aged type 2 diabetes.
