早期液体复苏对烧伤大鼠淋巴液TNF-α、IL-6、IL-8水平的影响

摘　　要：目的:研究早期液体复苏对严重烫伤大鼠淋巴液中TNFα、IL6、IL8水平的影响并探讨其与形态学改变的关系。方法:随机将42只成年雄性Wistar大鼠分为烧伤早期补液组(A组)、烧伤延迟补液组(B组)和对照组(C组)。在A组和B组动物背部造成30%TBSAⅢ度烫伤,利用倒置显微镜及录像系统观察大鼠肠系膜淋巴管伤后6、24、48h的运动变化,计算淋巴管收缩频率。经乳糜池插管收集淋巴液,计算淋巴液流速,用放射免疫分析法检测淋巴液TNFα、IL6、IL8水平的变化。结果:A、B组淋巴管收缩频率降低,淋巴液流速升高。镜下见小肠绒毛中央乳糜管扩张。淋巴液中TNFα、IL6、IL8水平升高。A、B两组收缩频率、淋巴液流速、淋巴液中TNFα、IL6、IL8水平有统计学差异。结论:严重烫伤休克期大鼠淋巴管扩张,运动减少,但淋巴液流速升高,淋巴液中TNFα、IL6、IL8水平升高。早期补液能够改善淋巴循环,降低淋巴液中TNFα、IL6、IL8水平。Objective： To study the effects of early fluid resuscitation on tumor necrosis factor-or （TNF-α）, interleukin-6 （IL-6） and interleukin-8 （IL-8） levels in lymph of rats with burn shock and explore their relationship with morphological changes. Methods： Forty-two male adult Wistar rats were randomly divided into burn resuscitation group （group A, n=18）, burn non- resuscitation group （group B, n=18） and control （group C, n=6）. Animals in groups A and B were made third-degree burns. By inverted microscope and digital image system, the motive frequency of the rat mesenteric lymphatic vessels was observed, and the speed of lymph and the concentrations of TNF-α、IL-6、IL-8in lymph 6, 24, 48 hours after burn were measured. Results： In groups A and B, the contractile frequency of mesenteric lymphatic vessels decreased. Despite the increasing lymph speed, the levels of TNF-α、IL-6、IL-8 in lymph increased in groups A and B than in group C. The villi central lacteal of small intestine were extremely dilated in group B. Conclusions： In the serious burn shock stage, the motion of lymphatic vessels decreases and the levels of TNF-α, IL-6, IL-8 in lymph increase. Early fluid resuscitation can improve the lymph circulation and decrease the levels of TNF-α,IL-6, and IL-8 in lymph.
