股骨干骨折患者髓内针固定后早期骨髓脂肪入侵与血小板激活状态的研究

摘　　要：目的:研究股骨干骨折患者髓内针固定后早期骨髓脂肪入侵对血小板激活状态的影响。方法:将29例股骨干骨折后行带锁髓内针固定患者作为实验组,27例软组织创伤或接受其他手术的患者作为对照组。采用血凝块冰冻切片油红“O”染色查找游离脂肪,流式细胞术检测CD62P。实验组患者在骨折后24h内、术前及术后24h内共检测6次,对照组患者在术前及术后24h内检测4次。结果:①实验组骨折后及髓内针固定后24h内血中游离脂肪阳性率和CD62P测定值均高于对照组;②实验组髓内针固定后6h内血中游离脂肪阳性率和CD62P测定值均高于术前;③实验组各时段血中游离脂肪呈阳性者CD62P测定值高于阴性者。结论:股骨干骨折及髓内针固定可引起骨髓脂肪入侵并进一步导致血小板激活。Objective： To study the relation between platelet activation and bone marrow fat intravasation in early stage after femur fracture and intrameduallary nailing. Methods： The experimental group consisted of 29 patients accepting intrameduallary nailing after femur fracture, and control group included 27 patients experiencing soft tissue trauma or other operations. The frozen sections of blood clots were stained by oil red “O” to find free fat and CD62P was measured by flowcytometry to reflect platelet activation. The measurements were taken 6 times during perioperational period in experimental group, and 4 times in control group. Results： ① In experimental group, the positivity rate of free fat and value of CD62P at any time during perioperational period were higher than those in control group; ②Within 6 hours after operation, the positivity rate of free fat and value of CD62P increased dramatically in experimental group; ③In experimental group, value of CD62P was higher at any time in patients with free fat detected in their blood when compared with those without free fat.Conclusion： Femur fracture and intrameduallary nailing would lead to the intravasation of bone marrow fat which results in platelet activation.
